摘　　要：目的调查绵阳市三级医院临床护士职业性下肢静脉曲张患病现状及影响因素,以期为临床护士职业性防护措施提供参考依据。方法采用便利抽样法,于2018年6—10月对绵阳市5所三级医院1447名临床护士进行下肢静脉曲张评定及一般资料调查。采用单因素分析和二元logistic回归分析进行相关影响因素探析。结果28.47%的护士患有下肢静脉曲张,其中沉重感占19.64%,疼痛占15.82%,肿胀感占12.47%,色素沉着占17.32%,瘙痒占10.28%,其他占17.5%。Logistic回归分析显示,年龄(OR=6.200,95%CI=2.095~18.353)、工龄(OR=18.312,95%CI=6.251~53.646)、所在科室(急诊科)(OR=14.833,95%CI=1.356~162.214)、体质指数(OR=3.345,95%CI=1.675~6.683)、防护措施(OR=0.149,95%CI=0.051~0.433)、参加运动(OR=0.162,95%CI=0.052~0.501)等是临床护士下肢静脉曲张的主要影响因素(均P<0.05)。结论绵阳市三级甲等医院临床护士下肢静脉曲张发病率高,严重影响临床护士身心健康。医院管理者应根据影响因素采取针对性干预措施,强化职业性防护意识和防护措施,降低临床护士下肢静脉曲张发病率,促进护理职业健康发展。Objective To investigate the current status and influencing factors of occupational lower extremity varicose vein in clinical nurses in tertiary hospitals of Mianyang City,provide reference for clinical nurses’occupational protection measures.Methods The convenience sampling method was used to carry out the lower extremity varicose vein evaluation and general data investigation among 1447 clinical nurses in five tertiary hospitals in Mianyang City from June to October 2018.Univariate analysis and binary logistic analysis were used to explore and analyze the related influencing factors.Results 28.47%of nurses suffered from varicose veins of the lower extremities,of which heaviness accounted for 19.64%,pain accounted for 15.82%,swelling accounted for 12.47%,pigmentation accounted for 17.32%,pruritus accounted for 10.28%,and other accounted for 17.5%.Logistic regression analysis showed that age(OR=6.200,95%CI 2.095~18.35),length of service(OR=18.312,95%CI6.251~53.646),department(emergency department)(OR=14.833,95%CI 1.356~162.214),body mass index(OR=3.345,95%CI1.675~6.683),protective measures(OR=0.149,95%CI 0.051~0.433),participation in sports(OR=0.162,95%CI 0.052~0.501)were the main influencing factors of clinical nurses’lower extremity varices(all P<0.05).Conclusion Clinical nurses of tertiary hospitals in Mianyang City have a high incidence of varicose vein in the lower extremities,seriously affecting the physical and mental health of clinical nurses.The clinical managers should take targeted interventions according to the influencing factors,strengthen the occupational protection awareness and protective measures,reduce the incidence of varicose veins in the lower limbs of clinical nurses,and promote the healthy development of nursing occupation.
