摘 要:目的建立不同年龄组正常青中年人扩展高频(9-20kHz)测听各频率听阈的参考基线。方法检测20-60岁正常人114例(228耳)扩展高频听阈,男42例,女72例,按年龄每10岁分为一组,共分4组,使用丹麦尔听美Conera纯音听力计Otosuite分别对9-20kHz各频率进行听阈检测。应用SPSS软件中的Kruskal-Wallis test对不同年龄组间扩展高频听阈进行统计分析,以P<0.05为差异具有统计学意义。结果在9-20kHz,与其他三组相比,20-29岁组扩展高频听阈最低,9-16kHz各组间听阈相比差异有显著性(P<0.05)。30-39岁组在9-14kHz听阈低于40-49岁组及50-60岁组,差异有显著性(P<0.05)。50-60岁组扩展高频听阈最大。随着年龄的增长,听阈检出率逐渐下降,50-60岁组在18k、20kHz处的检出率为0%。结论正常人扩展高频测听的听阈随年龄、频率的增高而升高。利用扩展高频测听结果评价早期高频听力损害时,需要考虑年龄以及所测频率,应根据每个年龄段相对应的听阈参考基线进行比较。Objective To establish reference baselines for extended high frequency(9-20 kHz)audiometry in young and middle-aged normal adults.Methods A total of 114 subjects(228 ears)aged between 20 and 60 years(42 males and 72 females)were tested for thresholds at extended high frequencyies.Subjects were divided into four groups at 10-year intervals by age.Thresholds at 9-20 kHz were measured with a Conera Otosuite audiometer(Otometrics,Denmark)and the Kruskal Wallis test was used to analyze hearing thresholds of different age groups,with P<0.05 considered as statistically significant.Results For the 9-20 kHz range,thresholds were the lowest in the 20-29-year-old group(P<0.05).For the 9-14 kHz range,thresholds in the 30-39-year-old group was lower than in the 40-49 and 50-60-year-old groups(P<0.05).With the increase of age,threshold detection rate gradually decreased and fell to 0%at 18 and 20 kHz in the 50-60-year-old group.Conclusion Hearing thresholds increase with age and frequency in normal adults.Age and frequency must be considered when interpreting extended high frequency audiometry results in evaluating early high frequency hearing loss with comparison to appropriate reference baselines for the specific age group.
