摘　　要：目的分析可能影响低频下降型突发性聋预后的因素,初步探讨1000Hz阈值在评估低频下降型突发性聋预后中的临床意义。方法回顾性分析我院2017年1月至2019年2月收治的68例低频下降型突发性聋患者的临床资料,采用EXCEL2016进行数据收集,SPSS17.0对收集到的资料进行描述性分析及统计分析,对性别、年龄、病程、听力损失程度、是否伴有耳鸣、耳闷、眩晕、合并症(糖尿病、高血压等)、1000 Hz阈值、使用溶媒(糖水组、盐水组、混合组)等因素进行单因素分析、二分类Logistic多因素分析以了解患者的预后及其影响因素。结果68例低频突发性聋患者中,42例治愈,1例显效,13例有效,12例无效,总有效率82.4%。1000 Hz阈值>25dB HL是影响低频下降型突发性聋预后的危险因素。结论低频下降型突聋总体预后较好,可根据患者发病时1000 Hz阈值评估患者的预后,并给与积极治疗。Objective To study the clinical significance of threshold at 1000 Hz in the prognosis of low-frequency sudden sensorineural hearing loss in discussion over possible factors influencing low-frequency sudden sensorineural hearing loss.Methods Clinical data of 68 patients diagnosed with low-frequency sudden sensorineural hearing loss from January 2017 to February 2019 were retrospectively analyzed,including sex,age,symptom duration,severity of hearing loss,tinnitus,aural fullness,vertigo,comorbid diseases(hypertension,diabetes,etc.),threshold at 1000 Hz and treatment solutions(normal saline,glucose and mixed).Logistic regression analysis of binary classification was used to understand the correlation between various factors and prognosis.Results Hearing completely recovered in 42 patients,significantly improved in 1 patient,moderately improved in 13 patients and remained unchanged in 12 patients,yielding a total effectiveness rate of 82.4%.threshold>25dB HL at 1000 Hz was a risk factor for poor prognosis.Conclusion The prognosis of low-frequency sensorineural hearing loss is generally good and may be predicted by the threshold at 1000 Hz.Timely treatment is critical for these patients.
