尿道下裂术后中远期效果评价及研究进展

摘　　要：尿道下裂是小儿泌尿生殖系统常见疾病,需要根据不同病情和医生个人经验采用不同的手术方法进行矫治。长期以来,相关文献多以尿道下裂术后近期并发症的报道为主,对于中远期治疗效果的报道较少。本文对近年来发表的有关尿道下裂术后中远期随访的文献进行了回顾,对尿道功能、阴茎外观、性功能及性心理方面存在的问题进行总结,发现尿道下裂术后有很多患者出现中远期的下尿路症状,且多数症状与尿道下裂的严重程度呈正相关;患者术后对阴茎外观的关注度很高,主要表现在阴茎大小和残余阴茎下弯方面;术后患者性功能与阴茎外观的满意度、尿道下裂的严重程度等有关;尿道下裂程度越重,其存在心理问题的可能性越大,手术次数和患者对阴茎外观的满意程度对其性心理有显著影响。As one of the common pediatric diseases of genitourinary system,hypospadias may be repaired by various surgical approaches according to disease status and surgeon experiences.For a long time,early complications after hypospadias repair have been reported in the literature while there are few reports of middle/long-term efficacies.Here the authors reviewed its recent reports of middle/long-term postoperative follow-ups and summarized the outstanding issues of urethral function,penile appearance,sexual function and psychology.Most symptoms of lower urinary tract were positively correlated with its severity.Patients were generally concerned about penile appearance or size and recurrence of curvature.Sexual function was correlated with the satisfaction degree of penile appearance and the severity of hypospadias.Severe hypospadias had a higher incidence of potential psychological problems.And sexual psychology was affected by operative frequency and patient satisfaction with penile appearance.
