摘　　要：目的研究无原发疾病创伤性脾破裂患者行脾切除术后血小板计数(platelet count,PLT)变化情况。方法选取2018年2月至2019年6月在本院接受脾切除术的80例无原发疾病创伤性脾破裂患者作为研究对象,以随机数字表法分为两组,各40例。对照组实施脾切除术,实验组在对照组基础上加自体脾组织移植术,比较两组PLT时间分布情况、频数分布、术后稳定值以及术后峰值。结果两组术前及术后前3周PLT比较差异无统计学意义,术后第4周以及第8周PLT比较差异有统计学意义(P<0.05);两组最高值频数比较差异无统计学意义;两组术前以及术后峰值比较差异均无统计学意义;但两组术后稳定值比较差异有统计学意义(P<0.05)。结论无原发疾病创伤性脾破裂患者行脾切除术后PLT呈现规律性改变,可增加血栓形成率,对此,可辅以脾组织移植术,达到降低PLT以及预防术后并发症的目的。Objective To study the changes of platelet count(PLT)after splenectomy in patients with traumatic spleen rupture without primary disease.Methods 80 patients with primary traumatic traumatic spleen rupture who underwent splenectomy in our hospital from February 2018 to June 2019 were selected as study subject.They were divided into two groups according to random number table method,with 40 cases in each group.The control group were only treated splenectomy,and the experimental group were treated autologous spleen tissue transplantation on the basis of the control group.The distribution of PLT time,frequency distribution,postoperative stability and postoperative peak were compared between the two groups.Results There was no significant difference in PLT between the two groups before and 3 weeks after operation.There was significant difference in PLT between the 4th and 8th week after surgery(P<0.05).There was no statistically significant difference in the frequency of the highest value between the two groups;there was no statistically significant difference in the peak value between the two groups before and after operation;but there was statistically significant difference in the postoperative stable value between the two groups(P<0.05).Conclusion PLT in patients with traumatic splenic rupture without primary disease has regular changes after psectomy,which can increase the probability of thrombosis.This can be supplemented with spleen transplantation to reduce PLT and prevent postoperative complications.purpose.
