细致化护理干预在骨科截肢患者围手术期中的应用价值研究

摘　　要：目的探究细致化护理干预在骨科截肢患者围手术期中应用价值。方法选取河南省人民医院2016年3月—2019年3月收治的108例骨科截肢患者作为研究对象,采取随机双盲法将其分为观察组和对照组,对照组患者(54例)实施围手术期常规护理干预,观察组患者(54例)实施细致化护理干预措施,对比两组患者护理质量评分、住院时间、并发症情况及护理满意度,对比两组患者干预后视觉模拟评分(VAS)、匹兹堡睡眠指数(PSQI)评分、生活质量评分(SF-36),采取汉密尔顿焦虑(HAMA)及抑郁(HAMD)量表评估患者心理状态。结果观察组患者护理质量评分高于对照组,住院时间短于对照组,VAS及PSQI、SF-36评分优于对照组(P<0.05);干预前两组患者HAMA、HAMD评分相当(P>0.05),干预后各组评分较干预前均改善,观察组优于对照组(P<0.05);观察组并发症率为3.70%,低于对照组14.81%,满意率为96.30%,显著高于对照组83.33%(P<0.05)。结论细致化护理干预在骨科截肢患者围手术期中应用价值极高,患者恢复快,心理状态佳,并发症率低,安全性高,疼痛轻,满意度高,值得推广应用。Objective To investigate the application value of intensive care intervention in patients with orthopedic amputation.Methods Selecting108 patients with orthopedic amputation in the hospital from March,2016 to March,2019 were selected as the subjects,and they were divided into observation group and control group through randomized double blind method.54 cases in control group received routine nursing intervention,and 54 patients in observation group received nursing intervention measures.Quali⁃ty grade,length of hospital stay,complications and nursing satisfaction of two groups after intervention were compared.Visual ana⁃logue scale(VAS),Pittsburgh sleep index(PSQI)score and quality of life scores(SF-36)were compared,and anxiety(HAMA)and Hamilton depression scale(HAMD)were used to evaluate patients psychological status.Results The quality score was higher in the observed group than in the control group,the VAS and PSQI and the SF-36 were better than the control group(P<0.05).In the first two groups,the HAMA and HAMD scores were similar(P>0.05),and the subjects were improved after the intervention and the group was better than the control group(P<0.05).The incidence rate of the observed group was 3.70%,compared with 14.81%in the control group,and the satisfaction rate was 96.30%,significantly higher than 83.33%in the control group(P<0.05).Conclusion Detailed nursing intervention in patients with orthopaedic amputation perioperational period application value is extremely high,patients recover quickly,psychological state is good,complication rate is low,security is high,pain is light,and sat⁃isfaction is high.So it is worth popularization and application.
