摘 要:目的探讨气滞胃痛颗粒对功能性消化不良胃敏感性及精神状态的影响。方法32只SD大鼠随机分为正常组、模型组、气滞胃痛颗粒组、莫沙必利组,每组8只。除正常组外,其余各组大鼠采用碘乙酰胺灌胃法制备功能性消化不良大鼠模型。造模成功后气滞胃痛颗粒组、枸橼酸莫沙必利组分别给予气滞胃痛颗粒水溶液1.35g/(kg·d)、莫沙必利水溶液1.35mg/(kg·d)灌胃,正常组、模型组分别给予0.5ml/100g蒸馏水灌胃,各组均每日1次,连续14天。采用肌电和腹壁撤退反射(AWR)评分检测大鼠胃敏感性,强迫游泳实验和旷场实验检测大鼠精神状态,脑功能核磁共振成像检测干预前后大鼠脑区功能改变。结果与正常组比较,模型组胃内球囊压力为40、60、80mmHg时大鼠肌电均方根变化率、AWR评分上升,旷场实验运动总距离缩短、强迫游泳水中不动时间增加(P<0.05或P<0.01),主要表现为下丘脑、海马、中脑被盖部、嗅皮层局部一致性(Reho)减弱,杏仁核、下丘脑、背外侧前额叶、小脑后叶低频振荡幅度(ALEF)减弱。与模型组比较,气滞胃痛颗粒组40、60、80mmHg时肌电均方根变化率、AWR评分显著降低,运动总距离明显增加,水中不动时间缩短(P<0.05或P<0.01),杏仁核、海马、下丘脑、前扣带皮层、楔前叶Reho增强,杏仁核、海马、楔前叶ALEF增强;莫沙必利组各指标差异无统计学意义(P>0.05),中脑被盖部、脑桥Reho增强,海马ALFF增强。与莫沙必利组比较,气滞胃痛颗粒组40、60、80mmHg时大鼠肌电均方根变化率、AWR评分显著降低,活动总距离明显增加,水中不动时间明显缩短(P<0.05或P<0.01),杏仁核、海马、下丘脑Reho增强,杏仁核、下丘脑ALFF增强。结论气滞胃痛颗粒可改善功能性消化不良模型大鼠的胃高敏感及精神状态,表现为不同脑区的结构改变。Objective To explore the effects of Qizhi Weitong Granules(气滞胃痛颗粒)on gastric sensitivity and mental state of rats with functional dyspepsia.Methods A total of 32 SD rats were randomly divided into a normal group,a model group,a Qizhi Weitong Granules group,and a mosapride group,with 8 rats in each group.Except for the normal group,rats were given iodoacetamide gavage to prepare functional dyspepsia rat models.After successful modeling,the Qizhi Weitong Granules group was given Qizhi Weitong Granules aqueous solution 1.35 g/(kg·d),and the mosapride citrate group was given mosapride aqueous solution 1.35 g/(kg·d)respectively.The normal group and the model group were given 0.5 ml/100 g distilled water by gavage,once a day in each group for 14 consecutive days.Electromyography and abdominal wall withdrawal reflex(AWR)scores were used to detect the gastric sensitivity of rats,the forced swimming test and the open field test were used to detect the mental state of the rats,and the brain function NMR was used to detect the functional changes of the rat brain before and after intervention.Results Compared with the normal group,in the model group,when the intragastric balloon pressure was 40,60,80 mmHg,the change rate of myoelectric root-mean-square and AWR score increased,the total distance of exercise in the open field experiment was shortened,and the time of immobilization in the water of forced swimming increased(P<0.05 or P<0.01),mainly manifested as weakened regional homogeneity(Reho)in hypothalamus,hippocampus,midbrain tegment,and olfactory cortex,and weakened amplitude of low-frequency fluctuation(ALFF)in amygdala,hypothalamus,dorsolateral prefrontal lobe,posterior cerebellar.Compared with the model group,in the Qizhi Weitong Granules group,the myoelectric root-mean-square change rate and AWR score at 40,60,and 80 mmHg were significantly reduced,the total distance of exercise was significantly increased,and the immobilization time in the water was shortened(P<0.05 or P<0.01).The Reho in amygdala,hipp
