摘  要:目的探讨蜕膜细胞衰老与胎膜完整早产的关系。方法选取2018年2月至2018年12月在淮安市妇幼保健院产科分娩的胎膜完整早产(妊娠28~36周)孕妇20例作为研究组,同时选取同期足月(妊娠37~40周)无产兆及足月临产孕妇各20例作为对照组。比较三组孕妇蜕膜细胞透射电镜下超微结构及核因子κB(factor kappa-B, NF-κB)和白介素-6(IL-6)差异。结果足月无产兆组孕妇粗面内质网面积与处理子区面积平均百分比为(9.2±1.1)%,足月临产组为(11.8±1.7)%,胎膜完整早产组为(10.2±1.3)%,三组比较,差异有统计学意义(P <0.05)。足月临产和胎膜完整早产组蜕膜细胞核内染色质加深、固缩,内质网和线粒体数量减少,结构肿胀,空泡化。胎膜完整早产组NF-κBp65及IL-6比足月临产组及足月无产兆组明显增高,差异有统计学意义(P <0.05)。结论透射电镜下足月无产兆孕妇蜕膜细胞呈现正常细胞形态,而足月临产组及蜕膜完整早产组患者蜕膜细胞均出现不同程度的细胞衰老表现。Objective To explore the relationship between the senescence of decidual cells and the premature delivery of intact fetal membranes. Methods From February 2018 to December 2018, 20 pregnant women with complete preterm labor(28-36 weeks of gestation) in the obstetric department of Huaian Maternal and Child Health Hospital were selected as observation group, while 20 full-term(37~40 weeks of gestation) without threatened labor and 20 full-term parturients were selected as control group. The ultrastructure of decidual cells in three groups was compared under transmission electron microscopy. Results The average percentage of rough endoplasmic reticulum area and treatment sub area was(9.2±1.1)% in the group of full-term non labor omen,(11.8±1.7)% in the group of full-term labor, and(10.2±1.3)% in the group of preterm delivery with intact fetal membrane. The difference among the three groups was statistically significant(P < 0.05). NF-kBp65 and IL-6 in the preterm group with intact fetal membrane were significantly higher than those in the parturient group and the asymptomatic group(P < 0.05). Conclusion Under transmission electron microscopy, decidual cells of full-term non-threatened patients showed normal cell morphology, while decidual cells of full-term parturient group and complete preterm delivery group showed different degrees of cell aging.
