芪贞银翘汤对2型糖尿病肾脏疾病患者肾脏功能的影响

摘　　要：目的:探讨芪贞银翘汤对2型糖尿病肾脏疾病(DKD)患者肾脏功能的影响。方法:收集2015年1月至2018年12月92例2型DKD住院患者,随机分为干预组及对照组。对照组予以常规降糖治疗和合并症常规治疗,干预组在对照组治疗基础之上加用芪贞银翘汤,连续治疗24 w,比较两组治疗前后尿白蛋白/肌酐比值(UACR)、血清及尿肝型脂肪酸结合蛋白(liver-type fatty acid-binding protein,L-FABP)、肝肾功能、血脂等生化指标的变化。结果:治疗24 w后,与治疗前比较,干预组UACR、尿L-FABP水平和中医症状积分均明显降低(P<0.05),而对照组仅中医症状积分明显降低(P<0.05);治疗后,与对照组比较,干预组UACR、尿L-FABP和中医症状积分均明显下降(P<0.05)。干预组治疗有效率(80%)明显高于对照组(11%)(P<0.05)。多重线性回归分析结果显示:芪贞银翘汤是UACR(β=0.071)、尿L-FABP(β=-0.064)水平下降的独立影响因素。结论:芪贞银翘汤可改善2型DKD患者UACR及尿L-FABP水平,可能是DKD患者的有效治疗药物。Objective:To explore the effect of Qizhen Yinqiao Decoction on the renal function in patients with diabetic kidney disease(DKD).Methods:From January 2015 to December 2018,92 patients with type 2 DKD from inpatients department were collected and randomly divided into the intervention group and the control group.Patients in the control group were treated with the conventional hypoglycemic and complication therapy,while patients in the intervention group were treated with the conventional hypoglycemic therapy and Qizhen Yinqiao Decoction for 24 weeks.The changes of UACR,plasma or urinary liver-type fatty acid-binding protein(L-FABP),liver and kidney function,blood lipid and other biochemical indexes were compared between the two groups before and after treatment.Results:After 24 weeks of treatment,compared with before treatment,the traditional Chinese medicine(TCM)symptom score was decreased in the control group(P<0.05)while the UACR,urinary L-FABP and TCM symptom score in the intervention group were significantly decreased(P<0.05).Compared with the control group,UACR,urinary L-FABP and TCM symptom score in the intervention group were significantly decreased(P<0.05).Compared with the control group,the effective rate in the intervention group was significantly increased(11%)(P<0.05).The multiple linear regression analysis showed that the application of Qizhen Yinqiao Decoction was an independent influencing factor for the decrease of UACR(β=0.071)and urine L-FABP(β=-0.064).Conclusion:Qizhen Yinqiao Decoction can improve the levels of UACR and urinary L-FABP in patients with DKD and type 2 diabetes,which may be an effective therapeutic drug for DKD patients.
