癌症合并2型糖尿病患者低血糖认知现状及影响因素分析

摘　　要：目的调查癌症合并2型糖尿病患者低血糖认知情况,分析影响因素,为临床低血糖健康宣教提供科学依据。方法选取2018-06-01-2019-04-30山东省肿瘤医院120例癌症合并2型糖尿病患者进行问卷调查,内容包括一般资料及低血糖认知情况,并分析低血糖认知现状的影响因素。结果回收有效问卷97份。97例合并糖尿病癌症患者低血糖认知知识调查问卷平均得分为(11.86±3.35)分,良好44例,一般33例,差20例。16项问题中,12项问题正确率>60%,有4项问题回答情况相对较差。单因素分析结果显示,不同文化程度(t=6.07,P<0.001)、不同职业(t=7.31,P<0.001)和不同月收入(t=4.37,P=0.02)合并糖尿病癌症患者低血糖认知情况差异有统计学意义。多因素Logistic回归分析结果显示,文化程度(t=2.293,95%CI为1.129~2.709,P=0.024)为合并糖尿病癌症患者低血糖认知的影响因素。结论合并糖尿病癌症患者对低血糖认知存在不足,文化程度、职业和月收入等因素影响其认知水平,护理人员应针对不同影响因素提出不同护理干预措施,提高患者认知水平,减少低血糖等并发症,提高患者生活质量。OBJECTIVE To investigate the cognition of hypoglycemia in cancer patients with type 2 diabetes mellitus(T2DM)and analyze the influencing factors,so as to provide scientific basis for clinical hypoglycemia health education.METHODS A total of 120 patients in Shandong Cancer Hospital from June 1,2018 to April 30,2019 were selected for questionnaire survey,including general data and hypoglycemia cognition of cancer patients with type 2 diabetes mellitus,and the influencing factors of hypoglycemia cognition were analyzed.RESULTS A total of 87 valid questionnaires were collected.The average score of the questionnaire was 11.86±3.35,44 cases were good,33 cases were fair and 20 cases were poor.Among the 16 questions,the correct rate of 12 questions was more than 60%,and the answers of 4 questions were relatively poor.Univariate analysis showed that there were significant differences in cognition of hypoglycemia among patients with diabetes mellitus and cancer with different educational level(t=6.07,P<0.001),different occupation(t=7.31,P<0.001)and different monthly income(t=4.37,P=0.02).Multivariate logistic regression analysis showed that education level(t=2.293,95%CI:1.129-2.709,P=0.024)was the influencing factor for cognition of hypoglycemia in patients with diabetes mellitus and cancer.CONCLUSIONS The cognition of hypoglycemia in cancer patients with diabetes mellitus is insufficient.Factors such as education level,occupation and monthly income affect their cognitive level.Nursing staff should put forward different nursing intervention measures according to different influencing factors to improve the cognitive level of patients,reduce complications such as hypoglycemia,and improve the quality of life of patients.
