摘 要:目的了解铁岭市手足口病病原监测结果,为手足口病防控提供科学依据。方法采用实时荧光定量RT-PCR方法对2011~2017年送检的1 587份标本同时进行肠道病毒(EV)通用型、肠道病毒71型(EV-A71)、柯萨奇病毒A16型(CV-A16)核酸检测,并对监测数据进行分析。结果 1 587份标本中,EV通用型核酸阳性1 107份,总阳性率为69.75%,其中EV-A71型374份,占33.79%(374/1 107),CV-A16型409份,占36.95%(409/1 107),其他EV324份,占29.27%(324/1 107)。2011~2017年各年手足口病病毒核酸阳性率差异有统计学意义(P<0.01)。2011、2012、2014、2017年EV-A71型为优势病原,占总阳性的构成比分别为59.62%、48.80%、57.39%、75.71%;2016年CV-A16型为优势病原,占总阳性的构成比为84.97%。铁岭市手足口病检出高峰主要集中在7~8月,阳性病例主要集中在5岁以下儿童,占总阳性病例的91.24%(1 010/1 107),性别比为1.46∶1。结论铁岭市2011~2017年手足口病病原分型呈动态变化,病原优势血清型别不固定。Objective To understand the surveillance results of hand-foot-and-mouth disease(HFMD) pathogen in Tieling city,so as to provide scientific basis for the prevention and control of HFMD. Methods Real-time fluorescent quantitative RT-PCR was used to detect the enterovirus(EV) universal type,EV-A71 and CV-A16 nucleic acids in 1 587 samples from 2011 to 2017,and the monitoring data were analyzed. Results Among the 1 587 samples,1 107 samples were positive for EV universal type nucleic acid,the total positive rate was 69.75%.And 33.79% were EV-A71(374/1 107),36.95% were CV-A16(409/1 107)and 29.27% were other EVs(324/1 107).There was significant difference in the annual positive rate of HFMD virus nucleic acid detection in Tieling city from 2011 to 2017(P<0.01).In 2011,2012,2014 and 2017,EV-A71 was the dominant pathogen,and accounted for 59.62%,48.80%,57.39% and 75.71% respectively;while CV-A16 was the dominant pathogen in 2016 and accounted for 84.97%.In 2013 and 2015,other EVs accounted for a relatively high proportion of pathogens and was 45.52% and 60.56%.The detection peak of HFMD positive cases in Tieling city was from July to August.The positive cases were concentrated in children under 5 years old,accounting for 91.24% of the total positive cases.The ratio of male to female was 1.46:1.There were significant differences in the positive rates of EV-A71,CV-A16 and other EVs among different age groups(P<0.05). Conclusion The pathogenic types of HFMD in Tieling city show dynamic changes from 2011 to 2017,and the dominant serotypes of pathogens are not fixed.
