摘  要:目的探讨"骨隧道内骨迁移技术"在关节镜辅助下SchazkerⅡ、Ⅲ型胫骨平台骨折微创手术中的治疗效果。方法将21例行关节镜辅助下微创治疗的SchazkerⅡ、Ⅲ型胫骨平台骨折患者随机分为骨迁移技术组(A组,9例)和传统微创手术组(B组,12例),比较两组手术治疗后的临床效果。结果21例患者随访时间8~32个月,骨折均愈合,无明显骨折复位丢失。两组手术时间、切口总长度、膝关节功能评分比较差异无统计学意义(P>0.05);与B组相比,A组人工骨植骨量减少,自体骨植骨量增加,骨折愈合时间缩短(P<0.05)。结论"骨隧道内骨迁移技术"在关节镜辅助下SchazkerⅡ、Ⅲ型胫骨平台骨折的微创手术中临床疗效满意,显著减少了人工骨的植骨量,在自体骨移植及骨折愈合方面具有明显优势。Objective To investigate the clinical efficacy of"bone migration technique in bone tunnel"in minimally invasive surgery of SchazkerⅡandⅢtibial plateau fractures assisted by arthroscopy.Methods Twenty-one patients with SchazkerⅡandⅢtibial plateau fractures underwent minimally invasive surgery assisted by arthroscopy were randomly divided into two groups of A(9 cases)and B(12 cases).The patients in group A accepted"bone migration technique in bone tunnel",while those in group B were treated with conventional minimally invasive surgery.The clinical outcomes after surgery were compared between the two groups.Results Twenty-one patients were followed up for 8-32 months,which showed that the fractures were healed without obvious loss of fracture reduction.There was no statistical difference in the operation time,total length of incision and score of knee function between groups of A and B(P>0.05).Compared with group B,the amount of artificial bone graft was decreased,the amount of autologous bone graft was increased and the healing time of fracture was shortened in group A(P<0.05).Conclusion The clinical efficacy of"bone migration technique in bone tunnel"in minimally invasive surgery of SchazkerⅡandⅢtibial plateau fracture assisted by arthroscopy is satisfactory,which may significantly reduce the amount of artificial bone graft and has the obvious advantages of increasing autologous bone graft and shortening healing time of fracture.
