摘  要:目的探讨重组人酸性成纤维细胞生长因子(recombinant human acid fibroblast growth factor,rhaFGF)温敏凝胶及Rg3水凝胶对大鼠烫伤创面促愈合的作用机制。方法用SD大鼠建立烫伤模型,分别给予20μg/mL rhaFGF-泊洛沙姆温敏凝胶及235μg/cm2 Rg3-卡波姆水凝胶治疗24 d,并以相应的凝胶基质为对照,分别于首次给药后1、7、19、24 d进行动物大体观察并计算创面愈合率;利用HE、Masson染色观察皮肤组织的重构;利用免疫组化(immunohistochemical,IHC)试验检测炎症相关因子IL-6、IL-10、IL-1β及细胞外基质蛋白CollagenⅢ和α-SMA的表达。结果在愈合面积及愈合率方面,rhaFGF及Rg3凝胶治疗组均优于对照组(P<0.05);在组织病理学上,两个治疗组在组织致密度及空化现象上优于对照组;在CollagenⅢ表达上,两个治疗组均高于对照组(P<0.01),但在α-SMA的表达上,rhaFGF凝胶治疗组高于对照组(P<0.05),而Rg3凝胶治疗组与对照组差异无统计学意义(P>0.05);在炎症因子表达方面,两个治疗组在7 d时均可降低IL-6的表达(P<0.05),且Rg3凝胶治疗组降低了IL-1β的表达(P<0.01),升高了抑炎因子IL-10的表达(P<0.01)。结论rhaFGF-泊洛沙姆温敏凝胶与Rg3-卡波姆水凝胶均可促进烫伤创面愈合。rhaFGF-泊洛沙姆温敏凝胶在早期可通过促进成纤维细胞增殖及调控炎症发挥作用,而Rg3-卡波姆水凝胶在愈合过程中通过调控创面炎症微环境来促进创面愈合。Objective To investigate the mechanism of thermosensitive gel and Rg3 hydrogel of recombinant?human acid fibroblast growth factor(rhaFGF)in promoting the wound healing of scalding in SD rats.Methods SD rat model of scalding was established and treated with 20μg/mL rhaFGF-Poloxamer thermosensitive gel and 235μg/cm^2 Rg3-Carbomer hydrogel for 24 d respectively,using the corresponding gel matrix as controls.The animals were subjected to general observation 1,7,19 and 24 d after the first treatment,while the wound healing rate was calculated.HE and Masson staining were used to observe the remodeling of cutaneous tissue in skin wounds.The expressions of IL-6,IL-10,IL-1β,CollagenⅢandα-SMA were determined by immunohistochemical assay.Results Both rhaFGF and Rg3 groups were superior to the control groups in the healing area,healing rate,tissue density and cavitation.Both the expression levels of CollagenⅢin rhaFGF and Rg3 groups were significantly higher than those in control groups(P<0.05).However,the expression level ofα-SMA in rhaFGF group was significantly higher than in that control group(P<0.01),while that in Rg3-gel group showed no significant difference with that in control group(P>0.05).The expression level of IL-6 decreased 7 d after treatment with rhaFGF and Rg3(P<0.05).However,in Rg3 group,the expression level of IL-1βdecreased(P<0.01)while that of IL-10 increased(P<0.01).Conclusion Both the rhaFGF thermosensitive gel and Rg3 hydrogel promoted the wound healing of scalding.The rhaFGF gel improved the wound healing by promoting the proliferation of fibroblasts and regulating inflammation at early stage of scalding However,Rg3 hydrogel promoted the wound healing by regulating the microenvironment of wound inflammation.
