摘 要:目的分析血液透析联合血液灌流治疗对慢性肾衰竭患者临床疗效、肾功能及炎性因子的影响。方法回顾性分析2017年3月—2018年3月88例慢性肾衰竭患者的临床资料,按照治疗方法分为联合组和对照组,每组44例。对照组采用血液透析治疗,联合组采用血液透析联合血液灌流治疗。记录和比较2组的临床疗效,以及治疗前后β2微球蛋白(β2-MG)、血肌酐(SCr)、尿素氮(BUN)、白介素-1(IL-1)、IL-6、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、甲状旁腺素(PTH)、同型半胱氨酸(Hcy)及血磷水平变化。结果治疗后,2组β2-MG、SCr、BUN、IL-1、IL-6、hs-CRP、TNF-α、PTH、Hcy及血磷水平较治疗前显著下降,且联合组均显著低于对照组(P<0.05)。联合组临床总有效率显著高于对照组(P<0.05)。结论血液透析联合血液灌流治疗可综合二者优势,对清除慢性肾衰竭患者体内毒素、改善肾功能、控制炎症反应及调节Hcy和PTH水平的效果显著。Objective To analyze the effect of hemodialysis combined with hemoperfusion on clinical efficacy,renal function and inflammatory factors in patients with chronic renal failure(CRF).Methods The clinical data of 88 patients with CRF from March 2017 to March 2018 were retrospectively analyzed,and they were divided into combination group(n=44)and control group(n=44)according to different treatment methods.The control group was treated with hemodialysis,and the combination group was treated with hemodialysis combined with hemoperfusion.The clinical effects of the two groups were recorded and compared,as well as the changes ofβ2 microglobulin(β2-MG),serum creatinine(SCr),urea nitrogen(BUN),interleukin-1(IL-1),IL-6,hypersensitive C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α),parathyroid hormone(PTH),homocysteine(Hcy)and blood phosphorus levels before and after treatment.Results After treatment,β2-MG,SCr,BUN,IL-1,IL-6,hs-CRP,TNF-α,PTH,Hcy and blood phosphorus levels in the two groups were significantly lower than those before treatment,and the above indicators were significantly lower in combination group than in the control group(P<0.05).The total effective rate of the combination group was significantly higher than that of the control group(P<0.05).Conclusion Hemodialysis combined with hemoperfusion therapy can combine their own advantages,and has significant effects on removing toxins from patients with CRF,improving renal function,controlling inflammation,and regulating Hcy and PTH levels.
