精索内静脉近心端-腹壁下静脉转流术对左精索静脉曲张合并肾静脉胡桃夹综合征的影响

摘　　要：目的探讨精索内静脉近心端-腹壁下静脉转流术治疗左精索静脉曲张合并肾静脉胡桃夹综合征患者的临床效果。方法回顾性分析我院2017年1月至2018年1月收治的左精索静脉曲张合并肾静脉胡桃夹综合征患者99例,其中43例行显微镜下精索内静脉结扎术为对照组,56例行精索内静脉近心端-腹壁下静脉转流术为试验组。比较两组手术前后的Dr、尿常规以及精子情况。结果试验组术后6个月及1年的Dr均显著低于对照组(P<0.05)。随访1年,试验组的尿常规结果显著优于对照组(P<0.05)。术后6个月,试验组的精子畸形率显著低于对照组,精子浓度及精子活力均显著高于对照组(P<0.05)。结论精索内静脉近心端-腹壁下静脉转流术治疗左精索静脉曲张合并肾静脉胡桃夹综合征患者的效果显著,安全性高。Objective To explore the clinical effect of proximal-inferior epigastric venous bypass of internal spermatic vein in the treatment of patients with left varicocele complicated with renal vein nutcracker syndrome.Methods The clinical data of 99 patients with left varicocele complicated with renal vein nutcracker syndrome admitted to our hospital from January 2017 to January 2018 were analyzed retrospectively.Among them,43 patients treated with intra-spermatic vein ligation under microscope were set as control group,and 56 patients treated with proximal-inferior epigastric venous bypass of internal spermatic vein were set as experimental group.The Dr,urine routine and sperm before and after surgery were compared between the two groups.Results The Dr at 6 months and 1 year after surgery of experimental group were significantly lower than those of control group(P<0.05).Following up for 1 year,the urine routine result of experimental group was significantly better than that of control group(P<0.05).6 months after surgery,the sperm deformity rate of experimental group was significantly lower than that of control group,and the sperm concentration and sperm motility were significantly higher than those of control group(P<0.05).Conclusions Proximal-inferior epigastric venous bypass of internal spermatic vein in the treatment of patients with left varicocele complicated with renal vein nutcracker syndrome has significant effect and high safety.
