摘 要:目的:研究颅内压监护仪测定颅内压对行呼吸机吸氧的颅脑损伤患者术后利尿剂甘露醇用药剂量及预后的预测价值。方法:选取在医院住院并行开颅手术的150例颅脑损伤患者;按随机数表法将其分为观察组和对照组,每组75例,观察组患者术后采取连接颅内检测仪并调整脱水剂使用剂量检测颅内压情况,对照组患者采取常规检测方法,比较两组患者的重残率、病死率、卡氏功能量表(KPS)评分和生活质量之间的差异。结果:观察组患者的重残率以及病死率显著低于对照组,差异有统计学意义(x2=4.812,x2=4.131;P<0.05);经过治疗后,两组患者的KPS评分均显著升高,且观察组患者的KPS评分显著高于对照组,差异有统计学意义(t=49.434,P<0.05);观察组患者的躯体功能、心理功能、社会功能及物质生活状态评分显著高于对照组(t=8.973,t=10.800,t=4.665,t=8.599;P<0.05)。结论:颅内压监护仪测定颅内压对行呼吸机吸氧的颅脑损伤患者术后甘露醇用药剂量及预后具有科学的指导意义。Objective:To study the predictive value of measuring intracranial pressure by intracranial pressure monitor on the usage dose postoperative diuretic(mannitol)and the prognosis of patient with craniocerebral injury who underwent ventilator for oxygen uptake.Methods:150 patients with craniocerebral injury who underwent craniotomy in hospital were selected as the research objects.And they were randomly divided into observation group and control group according to the random number table,with 75 patients in each group.The intracranial detector was connected to detect intracranial pressure after surgery in observation group,while the control group adopted routine detection method.And the differences of the rate of severe disability,mortality,Karnofsky performance status(KPS)score and the quality of life between two groups were compared further.Results:The rate of severe disability and mortality of patients in observation group were significantly lower than those in control group(x2=4.812,x2=4.131,P<0.05).After treatment,the KPS scores of patients both two groups were significantly increased,and the KPS scores of patients in observation group were significantly higher than those in control group(t=49.434,P<0.05).The score of quality of life of observation group was significantly higher than that of control group(t=8.973,t=10.800,t=4.665,t=8.599,P<0.05).Conclusion:The measurement of intracranial pressure by intracranial pressure monitor has scientific guiding significance for the postoperative usage dose of mannitol and the prognosis of patients with craniocerebral injury who undergo ventilator for oxygen uptake.
