摘  要:目的探讨NADPH氧化酶p22phox亚基C242T基因多态性与中国朝鲜族糖尿病足(DF)患者足背动脉硬化(FDA)的相关性。方法选取2017年5月至2019年5月于我院内分泌科及手足外科住院的朝鲜族DF患者238例,彩色多普勒超声检查患者FDA情况,将其中203例并发FDA患者分为无足溃疡组(NFDAU,n=50)和足溃疡组(FDAU,n=153),检测生化指标。应用聚合酶链反应⁃限制性片段长度多态性技术对NADPH氧化酶p22phox亚基C242T位点进行基因型分析。结果两组NADPH氧化酶p22phox亚基C242T位点上,各基因型及等位基因频率差异无统计学意义;CC基因与CT及TT基因基因频率差异有统计学意义(P<0.05);Logistic回归分析显示,NADPH氧化酶p22phox亚基C242T基因多态性、DF病程、吸烟、饮酒及高尿酸是FDAU发生的危险因素;纯合子CC基因型FDAU的风险高于杂合子CT及纯合子TT基因型患者。结论NADPH氧化酶p22phox亚基C242T基因多态性与中国朝鲜族DF并发FDA相关,其中CC纯合子基因是DF患者并发FDAU的遗传学危险因素。Objective To investigate the correlation of the NADPH oxidase subunit p22phox gene C242T polymorphism with foot dorsal arteriosclerosis(FDA)in Chinese Korean patients with diabetic foot(DF).Methods A total of 238 Chinese Korean patients with DF hospitalized in the Department of Endocrinology and Hand And Foot Surgery in our hospital were selected from May 2017 to May 2019.The FDA was evaluated by color Doppler ultrasound.The patients with DF complicated with FDA were divided into two groups:non dorsal artery ulcer group(NFDAU,n=50)and dorsal foot artery ulcer group(FDAU,n=153).All the subjects underwent biochemical tests.The genotypes of NADPH oxidase p22phox subunit C242T were analyzed by polymerase chain reaction restriction fragment length polymor⁃phism.Results There is no significantly difference in genotypes and alleles in NADPH oxidase subunit p22phox gene 242 between the two groups(P>0.05).The distributions of CC genotype vs CT and TT genotype in NADPH oxidase subunit p22phox gene 242 were significantly different between the two groups(P<0.05).Logistic regression analysis showed that NADPH oxidase subunit p22phox gene C242T poly⁃morphism,DF duration,smoking,drinking and hyperuricemia were risk factors for FDAU.The risk of FDAU development was higher in CC homozygous genotype than in CT heterozygous and TT homozygous genotypes.Conclusion The NADPH oxidase subunit p22phox gene C242T polymorphism was associated with FDA in Chinese Korean patients with DF.CC homozygous genotype was a genetic risk factor for FDAU in patients with DF.
