摘  要:目的观察白介素22(IL⁃22)及其受体(IL⁃22R1)在DM慢性皮肤溃疡(DU)边缘皮肤组织中的表达特征,探讨其与DU愈合的关系。方法选取2013年8月至2015年8月于广州中医药大学第二附属医院内分泌科住院的T2DM患者9例,取自足部溃疡边缘皮肤组织(DU组);另选取皮肤科住院的非DM慢性皮肤溃疡组织12例患者皮肤溃疡存档蜡块,取自下肢皮肤组织(NDU组)为研究对象,以及骨科10例患者手术中多余下肢正常皮肤组织(NC组)。HE染色观察皮肤组织学变化,免疫组织化学法观察IL⁃22及IL⁃22R1定位和表达,RT⁃PCR检测NC、DU组IL⁃22及IL⁃22R1 mRNA表达。结果与NC组比较,NDU、DU组表皮层增厚(P<0.01),真皮层变薄(P<0.01)。DU组IL⁃22呈弱阳性和阳性表达,IL⁃22R1呈弱阳性表达。与NC组比较,NDU、DU组IL⁃22、IL⁃22R1细胞阳性率降低(P<0.01)。DU组IL⁃22细胞阳性率高于NDU组,IL⁃22R1细胞阳性率低于NDU组(P<0.01)。DU组IL⁃22 mRNA、IL⁃22R1 mRNA表达低于NC组(P<0.01)。结论IL⁃22及IL⁃22R1参与DU发生及难愈合机制,具体信号通路及调节因子需进一步研究加以证实。Objective To assess the expressions of IL⁃22 and IL⁃22R1 in chronic diabetic ulcer(DU),chronic non⁃diabetic ulcer(NDU)and normal(NC)skin tissue.Methods 9 chronic DU cases,12 NDU cases and 10 NC cases from The Second Affiliated Hospital of Guangzhou University of Chinese Medicine were selected from August 2013 to August 2015.Histological examination of the specimens was performed.Immunohistochemistry was used to observe the localization and expression of IL⁃22 and IL⁃22R1.RT⁃PCR was used to detect the mRNA expressions of IL⁃22 and IL⁃22R1.Results Compared with NC group,the thickness of epidermis and dermis in NDU and DU groups increased(P<0.01).In DU group,IL⁃22 showed weak positive or positive expression,while IL⁃22R1 showed weak positive expression.Compared with NC group,the positive rates of IL⁃22 and IL⁃22R1 cells in NDU and DU groups decreased(P<0.01).The positive rate of IL⁃22 cells in DU group was higher than that in NDU group,while the positive rate of IL⁃22R1 cells was lower than that in NDU group(P<0.01).The expressions of IL⁃22 mRNA and IL⁃22R1 mRNA in DU group were lower than those in NC group(P<0.01).Conclusion IL⁃22 and IL⁃22R1 might be involved in the mechanism of wound non⁃healing in chronic DU,but the related signal pathways and regulatory factors still need further study.
