摘　　要：目的:探讨自制中药包湿热敷促进动静脉内瘘成熟的效果。方法:纳入2018年9月—2020年1月江西省樟树市人民医院内一科接受单侧肢体动静脉内瘘首次手术患者60例,依据随机对照原则分为2组各30例。对照组接受功能锻炼;观察组在对照组基础上联合自制中药包湿热敷,20 min/次,2次/d,疗程3周。观察头静脉血流、内径变化情况和经济指标对比情况。结果:术后2周、4周、6周时,两组的头静脉血流、内径较手术结束时均有改善(P<0.05),且观察组的改善情况优于对照组(P<0.05)。观察组内瘘平均成熟天数短于对照组(P<0.05),且中药包、置管护理和平均导管相关感染治疗总体费用少于对照组(P<0.05)。结论:自制中药包湿热敷促进动静脉内瘘成熟的效果明显,还可以改善患者的经济指标。Objective:To investigate the effect of self-made Chinese medicine package wet hot compress in promoting the maturation of arteriovenous fistula.Methods:From September 2018 to January 2020,60 patients with unilateral limb arteriovenous fistula in the First Department of Internal Medicine,Zhangshu People’s Hospital,Jiangxi Province were included,and divided into two groups with 30 cases in each group according to the principle of random control.The control group received functional exercise;on the basis of the control group,the observation group was combined with self-made Chinese medicine package wet hot compress,20 min/time,2 times/D,and the course of treatment was 3 weeks.The blood flow of the cephalic vein,the change of inner diameter and the comparison of economic indicators were observed.Results:At 2,4,and 6 weeks after surgery,the blood flow and inner diameter of the cephalic vein in the two groups were improved compared with those at the end of the operation(P<0.05),and the improvement in the observation group was better than that in the control group(P<0.05).The average mature days of fistula in the observation group were shorter than those in the control group(P<0.05),and the overall cost of traditional Chinese medicine package,catheter care and mean catheter-related infection treatment in the observation group was lower than that in the control group(P<0.05).Conclusion:The self-made Chinese medicine package wet hot compress has obvious effect in promoting the maturation of arteriovenous fistula,and can also improve the economic indicators of patients.
