血浆DD和FDP对子痫前期的预测价值

摘　　要：目的探讨孕晚期联合检测血浆D-二聚体(DD)和纤维蛋白(原)降解产物(FDP)对子痫前期的预测价值。方法选取被确诊为子痫前期的126例单胎孕妇作为子痫前期组,其中重度子痫前期40例(重度子痫前期组)、轻度子痫前期86例(轻度子痫前期组)。以同期136名体检健康孕妇作为对照组。检测所有孕妇血浆DD和FDP水平。采用受试者工作特征(ROC)曲线评价孕晚期联合检测血浆DD和FDP水平对子痫前期的预测价值。结果子痫前期组孕晚期血浆DD、FDP水平显著高于对照组(P<0.05);重度子痫前期组孕晚期血浆DD及FDP水平与轻度子痫前期组比较,差异均无统计学意义(P>0.05)。ROC曲线分析结果显示,孕晚期血浆DD和FDP预测子痫前期的曲线下面积分别为0.855、0.869,最佳临界值分别为3.64 mg/L FEU、10.65μg/mL,敏感性分别为83.5%、86.7%,特异性分别为65.0%、60.0%。结论孕晚期血浆DD和FDP水平与子痫前期的发生密切相关,对子痫前期有一定预测价值。Objective To investigate the predictive value of the combined determination of plasma D-dimer and fibrinogen degradation product(FDP)in pre-eclampsia in the third trimester of pregnancy.Methods A total of 126 women with singleton pregnancy and pre-eclampsia,including 40 patients with severe pre-eclampsia and 86 patients with mild pre-eclampsia,were enrolled.A total of 136 healthy pregnant women were enrolled as control group.The plasma D-dimer and FDP were determined.Receiver operating characteristic(ROC)curve analysis was used to evaluate the predictive value of the combined determination of plasma D-dimer and FDP levels in preeclampsia.Results Plasma D-dimer and FDP levels in the pre-eclampsia group were higher than those in control group(P<0.05).Between severe and mild pre-eclampsia groups,there was no statistical significance in plasma D-dimer and FDP in late trimester of pregnancy(P>0.05).ROC curve analysis showed that the areas under curves of D-dimer and FDP in the diagnosis of pre-eclampsia were 0.855 and 0.869,and the optimal cut-off values were 3.64 mg/L FEU and 10.65μg/mL.The sensitivities were 83.5%and 86.7%,and the specificities were 65.0%and 60.0%.Conclusions Plasma D-dimer and FDP levels in the third trimester are closely related to the occurrence of preeclampsia,and have predictive values for pre-eclampsia.
