早期腹腔镜探查术在输卵管积脓中的应用

摘　　要：目的:探讨早期腹腔镜探查术在输卵管积脓患者中的应用。方法:回顾性分析我院收治的52例输卵管积脓并给予抗感染治疗+腹腔镜探查术的患者资料。根据治疗方案不同分为观察组和对照组,观察组入院后抗感染同时尽快行腹腔镜探查术,对照组行常规抗感染治疗无效后再行手术治疗。结果:观察组经过积极腹腔镜探查术和抗感染治疗后,总有效率为96.43%,对照组总有效率为95.83%,两组比较差异无统计学意义(P>0.05)。两组患者术中出血量比较,差异无统计学意义(P>0.05)。但观察组所用手术时间更短,分离困难和中转开腹率更低,差异均具有统计学意义(均P<0.05),术后腹痛消失时间、白细胞恢复正常时间、体温恢复正常时间、静滴抗生素时间、平均住院时间均显著短于对照组,差异均有统计学意义(均P<0.05)。结论:抗感染同时尽早行腹腔镜探查术可促进输卵管积脓患者的快速康复。Objective:To explore the application of early laparoscopic surgery in patients with salpingo empyema.Methods:The data of 52 patients with salpingo empyema who were treated with anti-infective therapy and laparoscopic exploration were analyzed retrospectively.The patients in the observation group received anti-infection and laparoscopic exploration as soon as possible after admission,while the patients in the control group received surgical treatment after the failure of routine anti-infection treatment.Results:After active laparoscopic exploration and anti-infection treatment,the total effective rate was 96.43%in the observation group and 95.83%in the control group,with no significant difference between the two groups(P>0.05).There was no significant difference in the amount of bleeding between the two groups(P>0.05).However,the time of operation was shorter,the rate of difficulty separation and conversion to laparotomy were lower in the observation group than those of control group(all P<0.05).The time for the abdominal pain to disappear,the time for white blood cells and body temperature to become normal,the time for intravenous antibiotics and the average length of hospital stay were all significantly shorter than the control group(all P<0.05).Conclusion:Laparoscopic exploration performed as early as possible could promote rapid recovery in patients with salpingo empyema.
