摘  要:目的探讨早期全负重训练在老年不稳定性股骨转子间骨折患者生物型全髋关节置换术中的应用。方法选择2016年12月至2018年12月青岛市市立医院收治的老年不稳定性股骨转子间骨折患者76例为研究对象,依据随机数字表法将入选患者分为对照组与观察组,每组各38例。两组患者均行生物型全髋关节置换术。对照组患者于术后6周进行负重训练,观察组患者于术后2周左右在耐受范围内完全负重。比较两组患者术后1个月、3个月和6个月视觉模拟评分法(visual analogue score,VAS)评分、Harris髋关节评分(Harris hip score,HHS)和日常生活活动(activity of daily living,ADL)量表评分。结果观察组患者术后1个月、3个月和6个月VAS评分均显著低于同期对照组(均P<0.05),HHS和ADL量表评分均显著高于同期对照组(均P<0.05)。结论早期全负重训练在老年不稳定性股骨转子间骨折患者生物型全髋关节置换术中具有良好的应用效果,可减轻患者疼痛,改善髋关节功能,提高患者ADL能力。Objective To explore the application of early total weight-bearing training in biological total hip replacement for elderly patients with unstable intertrochanteric fracture of femur.Method A total of 76 elderly patients with unstable intertrochanteric fracture of femur admitted to Qingdao Municipal Hospital from December 2016 to December 2018 were randomly divided into control group(38 cases)and observation group(38 cases)according to the random number table method.Biological total hip replacement was performed in both groups.Control group patients received weight-bearing training 6 weeks after operation,while observation group patients received full weight-bearing within the tolerance range of patients 2 weeks after operation.The visual analogue scale(VAS)scores,Harris hip score(HHS)and activity of daily living(ADL)scale scores were compared between the two groups at 1 month,3 months and 6 months after operation.Result The VAS score in observation group at 1 month,3 months and 6 months after operation were significantly lower than those of control group at the same period(all P<0.05),and the HHS and ADL scale scores were significantly higher than those of control group at the same period(all P<0.05).Conclusion Early total weight-bearing training has a good effect on elderly patients with unstable intertrochanteric fracture undergoing biological total hip replacement,which can relieve pain,improve hip function and improve ADL ability of patients.
