摘 要:目的探讨膝关节镜下骨折复位微创锁定加压钢板螺钉固定对膝关节周围骨折患者术后膝关节功能的影响。方法回顾性选取膝关节周围骨折患者72例,将行膝关节镜下骨折复位微创锁定加压钢板螺钉固定的40例患者设为观察组,将行切开复位内固定术的32例患者设为对照组。比较两组的治疗效果。结果两组的手术时间比较,差异无统计学意义(P>0.05);观察组的下床活动时间、住院时间、骨折愈合时间均短于对照组(P<0.05)。观察组的膝关节功能恢复优良率高于对照组(P<0.05)。观察组的步长、步速、步频均优于对照组(P<0.05)。术后1、2周,观察组的血清NPY、PGE2、SP水平均低于对照组(P<0.05)。结论膝关节镜下骨折复位微创锁定加压钢板螺钉固定应用于膝关节周围骨折患者中,能有效抑制疼痛介质水平,缩短下床活动时间、住院时间,促进骨折愈合,改善患者步态,促进膝关节功能恢复。Objective To investigate the effect of fracture reduction with minimally invasive locking compression plate and screw fixation under knee arthroscopy on postoperative knee joint function in patients with fractures around the knee.Methods A total of 72 patients with fractures around the knee were retrospectively selected.Forty patients underwent fracture reduction with minimally invasive locking compression plate and screw fixation under arthroscopy were set as observation group,and 32 patients underwent open reduction and internal fixation were set as control group.The therapeutic effects of the two groups were compared.Results There was no statistically significant difference in the operation time between the two groups(P>0.05);the time of ambulation,hospitalization time and fracture healing time of the observation group were shorter than those of the control group(P<0.05).The excellent and good rate of knee joint function recovery of the observation group was higher than that of the control group(P<0.05).The step length,walking speed and step frequency of the observation group were better than those of the control group(P<0.05).The serum NPY,PGE2 and SP levels at 1 and 2 weeks after operation of the observation group were lower than those of the control group(P<0.05).Conclusion Fracture reduction with minimally invasive locking compression plate and screw fixation under knee arthroscopy can effectively inhibit the level of pain medium,shorten the time of ambulation and hospitalization,promote fracture healing,improve the gait of patients,and promote the recovery of knee joint function.
