摘　　要：目的探讨保留距下关节跟骨丘部及后距下关节重建治疗严重陈旧性跟骨骨折的临床效果。方法回顾性分析2013年6月至2019年6月我院收治的40例严重陈旧性跟骨骨折患者的临床资料,患者均行保留距下关节跟骨丘部及后距下关节重建治疗。观察患者的治疗效果。结果所有患者均顺利完成手术,术后1、2、3个月的VAS评分明显低于术前(P<0.05)。术前及术后1、3个月,患者的Gissane角、Bohler角、丘部高度以及跟骨宽度比较,差异有统计学意义(P<0.05)。术后1、3个月,患者的AOFAS各项评分及总分均高于术前(P<0.05)。结论保留距下关节跟骨丘部及后距下关节重建术治疗严重陈旧性跟骨骨折患者,不仅可以减轻患者的疼痛,还可以促进肢体功能恢复,从而改善患者的预后。Objective To investigate the clinical effect of reserving calcaneal mound of subtalar joint and posterior subtalar joint reconstruction in the treatment of severe old calcaneal fracture.Methods The clinical data of 40 patients with severe old calcaneal fracture treated in our hospital from June 2013 to June 2019 were retrospectively analyzed,and all of them underwent reserving calcaneal mound of subtalar joint and posterior subtalar joint reconstruction.The therapeutic effects of the patients were observed.Results All the patients successfully completed the operation,and VAS scores at 1,2 and 3 months after operation were significantly lower than those before operation(P<0.05).There were significant differences in Gissane angle,Bohler angle,mound height and calcaneal width before operation and at 1,3 months after operation(P<0.05).At 1 and 3 months after operation,AOFAS scores and total scores of patients were higher than those before operation(P<0.05).Conclusion Reserving calcaneal mound of subtalar joint and posterior subtalar joint reconstruction in the treatment of severe old calcaneal fractures can not only reduce the pain of patients,but also promote the recovery of limb function,thus improving the prognosis of patients.
