和血明目片联合玻璃酸钠滴眼液对老年性白内障伴青光眼患者视功能及房水sCD44、TIMP-2水平的影响

摘　　要：目的探讨和血明目片联合玻璃酸钠滴眼液对老年性白内障伴青光眼患者视功能及房水可溶性黏附分子CD44(sCD44)、基质金属蛋白酶抑制因子-2(TIMP-2)水平的影响。方法选取我院2015年4月至2019年8月收治的150例老年性白内障伴青光眼患者,应用简单随机化分组法将其分为观察组和对照组,各75例。两组均行小梁切除术、超声乳化术,对照组给予玻璃酸钠滴眼液,观察组给予玻璃酸钠滴眼液+和血明目片,均治疗1个月。比较两组患者的视力恢复情况、眼压、疼痛程度、房水sCD44、TIMP-2水平及并发症发生情况。结果观察组的视力进步率明显高于对照组,视力减退率明显低于对照组(P<0.05)。治疗1个月后,观察组的眼压、VSA评分、房水s CD44、TIMP-2水平均明显低于对照组(P<0.05)。观察组的并发症总发生率明显低于对照组(P<0.05)。结论和血明目片联合玻璃酸钠滴眼液治疗老年性白内障伴青光眼,能改善患者视力,有效控制眼压,缓解眼部疼痛程度,保护视网膜神经节细胞,减少并发症的发生,其机制可能与降低sCD44、TIMP-2水平有关。Objective To investigate the effect Hexue Mingmu tablets combined with sodium hyaluronate eye drops on visual function and aqueous humor soluble adhesion molecule CD44(sCD44)and tissue inhibitor of matrix metalloproteinase-2(TIMP-2)levels in senile cataract patients with glaucoma.Methods A total of 150 patients with senile cataract and glaucoma admitted in our hospital from April 2015 to August 2019 were selected and divided into observation group and control group by simple randomized grouping method,with 75 cases in each group.Trabeculectomy and ultrasonic emulsification were performed in both groups,the control group was given sodium hyaluronate eye drops,while the observation group was given sodium hyaluronate eye drops+Hexue Mingmu tablets,all of which were treated for 1 month.The visual acuity recovery,intraocular pressure,pain,aqueous humor sCD44 and TIMP-2 levels and complications were compared between the two groups.Results The visual acuity improvement rate of the observation group was significantly higher than that of the control group,and the visual acuity decrease rate was significantly lower than that of the control group(P<0.05).After 1 month of treatment,the intraocular pressure,VSA score,aqueous humor sCD44 and TIMP-2 levels of the observation group were significantly lower than those of the control group(P<0.05).The total incidence of complications of the observation group was significantly lower than that of the control group(P<0.05).Conclusion Hexue Mingmu tablets combined with sodium hyaluronate eye drops in the treatment of senile cataract with glaucoma can improve patients’vision,it can effectively control intraocular pressure,relieve eye pain,protect retinal ganglion cells,and reduce complications,and the mechanism may be related to reducing sCD44 and TIMP-2 levels.
