摘　　要：目的探讨Scarf联合Akin截骨治疗拇外翻的临床疗效。方法采用Scarf联合Akin截骨治疗32例拇外翻患者。比较术前及末次随访时患足负重正位X线片中的拇外翻角(HVA)、第1、2跖骨间角(IMA)、第1跖骨远端关节面角(DMAA)变化;采用AOFAS前足评分系统评估临床疗效。结果患者均获得随访,时间12~16个月。全部患者拇外翻矫正效果明显,术后截骨处无延迟愈合或不愈合,患足内侧红肿、疼痛感消失,未见转移性跖痛和畸形复发。末次随访时,HVA、IMA、DMAA均较术前显著减小,AOFAS前足评分较术前明显升高,差异均有统计学意义(P<0.01)。结论Scarf联合Akin截骨能有效治疗拇外翻,可有效缓解患者疼痛,术后患足恢复良好,并发症少。Objective To investigate the clinical efficacy of Scarf combined with Akin osteotomy in the treatment of hallux valgus(HV).Methods The 32 patients with HV deformities were treated with Scarf osteotomy and Akin osteotomy.The changes of hallux valgus angle(HVA),the first and the second intermetatarsal angle(IMA)and the first distal metatarsal angle(DMAA)were compared between the preoperation and the last follow-up.AOFAS forefoot score system was performed to evaluate the clinical effect.Results All patients were followed up for 12~16 months.The corrective effect of HV was obvious in all patients.There was no delayed union or nonunion at the osteotomy site,no swelling and pain at the medial side of the foot,no metastatic metatarsal pain and deformity recurrence.At the last follow-up,HVA,IMA and DMAA were significantly reduced than the preoperation,AOFAS forefoot score was significantly increased than that before operation,the differences were statistically significant(P<0.01).Conclusions Scarf osteotomy combined with Akin osteotomy can effectively treat HV deformity,effectively relieve the pain of patients,and the recovery of affected foot is good,with few complications.
