摘　　要：目的探讨Ilizarov技术结合有限手术一期治疗重度马蹄内翻足合并小腿外旋畸形的疗效。方法采用足后内侧软组织松解、足三关节截骨、胫骨结节下内旋截骨、Ilizarov外固定器矫形术治疗15例重度马蹄内翻足合并小腿外旋畸形患者。术后缓慢调整足部外固定器,矫正残余的足部三维畸形。末次随访时采用AOFAS评分标准评估足踝关节功能。结果患者均获得随访,时间1~2(1.6±0.3)年。患者术后恢复满意,无针道及切口感染、血管神经损伤、截骨不愈合等并发症发生。末次随访时,患者足部畸形和小腿外旋矫正,行走功能良好,AOFAS评分较术前明显提高(P<0.05)。结论采用Ilizarov技术结合有限手术一期治疗重度马蹄内翻足合并小腿外旋畸形,可有效矫正小腿外旋及足部畸形,短期疗效满意。Objective To investigate the effect of Ilizarov technique combined with limited operation in one stage for treatment of severe equinovarus with external rotation deformity of lower leg.Methods Posteriomedial soft tissue release,triarticular osteotomy of foot,internal rotation osteotomy under tibial tubercle and Ilizarov external fixator were used to treat 15 patients with severe equinovarus combined with external rotation deformity of the lower leg.After the operation,the foot external fixator was adjusted slowly to correct the residual three-dimensional deformity of foot.At last follow-up,AOFAS score was used to evaluate the foot-ankle function.Results All patients were followed up for 1~2(1.6±0.3)years.The patient recovered satisfactorily after surgery,and there were no complications such as the pin site and incision infection,vascular and nerve injury and osteotomy nonunion.At the last follow-up,the foot deformity and external rotation of the lower leg were corrected,and the postoperative walking function was good.The AOFAS score was significantly increased than that before the operation(P<0.05).Conclusions The Ilizarov technique combined with limited operation in one stage for treatment of severe equinovarus with external rotation deformity of the lower leg can effectively correct the external rotation of the lower leg and foot deformity,which have satisfactory short-term results.
