摘　　要：目的探讨采用前外侧入路钢板内固定治疗老年胫骨Pilon骨折的疗效。方法采用前外侧入路钢板内固定治疗35例老年胫骨Pilon骨折患者。记录手术时间、术中出血量,术后根据Burwell-Charnley影像学评价标准判定骨折复位情况,末次随访时采用Teeny-Wiss踝关节功能评分标准评价疗效。结果手术时间60~110 min,术中出血量50~210 ml。术后骨折复位良好。患者均获得随访,时间6~18个月。末次随访Teeny-Wiss踝关节功能评分为85~100分,优26例,良5例,可4例。结论采用胫骨前外侧入路钢板内固定治疗老年胫骨Pilon骨折术中显露清楚,骨折复位良好,并发症少。Objective To investigate the effect of plate internal fixation for the treatment of elderly tibial Pilon fracture via anterolateral approach.Methods The 35 elderly patients with tibial Pilon fractures were treated with plate internal fixation via anterolateral approach.The operation time and blood loss during operation were recorded.The fracture reduction situation was determined according to Burwell-Charnley image evaluation criteria.Teeny-Wiss score was used to evaluate the ankle function at the last follow-up.Results The operation time was 60~110 min,the blood loss during operation was 50~210 ml.The fracture got good reduction at the postoperation.All the 35 cases were followed up for 6~18 months.At the last follow-up,the Teeny-Wiss score was from 85 points to 100 points,the result was excellent in 26 cases,good in 5,fair 4.Conclusions The treatment of elderly tibial Pilon fracture with plate internal fixation via anterolateral approach has the advantages of clear exposure,good fracture reduction and few postoperative complications.
