摘  要:目的:探讨3D打印技术在PaproskyⅢ型髋臼翻修中的临床应用效果。方法:选取2015年3月至2020年3月在我院进行髋臼翻修手术的PaproskyⅢ型髋臼缺受损患者80例为研究对象,按照患者住院编号奇偶进行分组,编号为偶数者纳入对照组(n=40),编号为奇数者纳入观察组(n=40),对照组给予常规全髋关节翻修术治疗,观察组给予3D打印技术进行修复治疗,对比两组患者髋关节恢复情况。结果:观察组骨折端复位优良率82.50%显著优于对照组62.50%(P<0.05);观察组手术时间短于对照组,术中出血量及输血量均显著少于对照组(P<0.05);术后观察组Harris评分、NRS评分改善情况明显优于对照组(P<0.05);术后两组曲髋活动度、外展活动度均显著增大,且观察组术后曲髋活动度、外展活动度改善情况及髋臼假体骨性覆盖率明显高于对照组(P<0.05);随访3个月,发现观察组并发症发生率2.5%显著低于对照组20.00%(P<0.05)。结论:3D打印技术在PaproskyⅢ型髋臼翻修中应用较佳,可降低术中出血量及术后并发症,促进术后恢复,值得推广。Objective:To explore the clinical application effect of 3D printing in Paprosky Ⅲ acetabular revision.Methods:80 patients with Paprosky Ⅲ acetabular defects treated with acetabular revision in the hospital between March 2015 and March 2020 were enrolled in this study.They were divided into control group(n=40,even numbers)and observation group(n=40,odd numbers)according to the odd or even hospitalization number.The control group was treated with routine total hip revision,and the observation group was given repair with 3D printing.Hip recovery of the two groups was compared.Results:The excellent and good rate of fracture end reduction in the observation group was 82.50%,significantly higher than 62.50% in the control group(P<0.05).The operation time,intraoperative blood loss and blood transfusion volume of observation group were significantly shorter/less than those of control group(P<0.05).After surgery,the improvement of Harris scores and NRS scores was significantly better in the observation group than in the control group(P<0.05).After surgery,ranges of hip joint flexion and abduction were significantly increased in both groups.Besides,improvement of ranges of hip joint flexion and abduction and bone coverage rate of acetabular prosthesis were significantly higher in observation group than in control group(P<0.05).3 months of follow-up found that the incidence rate of complications in observation group(2.5%)was significantly lower than that in control group(20.00%)(P<0.05).Conclusion:The application of 3D printing in Paprosky Ⅲ acetabular revision can significantly reduce intraoperative blood loss,intraoperative blood transfusion volume,pain and postoperative complications,and promote postoperative recovery.
