活血潜阳祛痰方治疗肥胖高血压伴左室肥厚的临床疗效观察

摘　　要：目的观察活血潜阳祛痰方治疗肥胖高血压伴左室肥厚的临床疗效。方法纳入上海中医药大学附属岳阳中西医结合医院心内科门诊及住院部的肥胖高血压伴左室肥厚病人90例,随机分为对照组及治疗组。对照组给予一般西医治疗方案,治疗组在对照组基础治疗上予口服活血潜阳祛痰方进行干预。两组疗程均为6个月。观察两组治疗3个月后和6个月后体质指数(BMI)、血压、糖脂代谢及氧化应激水平、左心室质量指数(LVMI)变化情况。结果治疗3个月后,治疗组BMI(24.99±1.17)kg/m2,较对照组(26.67±2.35)kg/m2下降(P<0.01);治疗6个月后,治疗组空腹胰岛素(FIN)为(12.54±5.42)mU/L、胰岛素抵抗指数(HOMA-IR)为2.94±1.74、总胆固醇(TC)为(3.95±0.64)mmol/L、收缩压和舒张压、LVMI(97.87±22.68)g/m2均较对照组下降(P<0.05);治疗组血清丙二醛(MDA)低于对照组(P<0.05),血清超氧化物歧化酶(SOD)水平高于对照组(P<0.05)。结论活血潜阳祛痰方可协同西药降低肥胖高血压伴左室肥厚病人BMI,改善糖脂代谢水平,降低血压,减轻氧化应激水平,从而改善病人左室肥厚程度。Objective To observe the clinical effect of Huoxue Qianyang Qutan Recipe on left ventricular hypertrophy in patients with obesity and hypertension.Methods Ninety patients were randomly divided into the control group and at same time the treatment group.Two groups were treated with western medicine treatment for 6 months,and at some time the treatment group was treated with Huoxue Qianyang Qutan Recipe for 6 months.The levels of body mass index(BMI),blood pressure,oxidative stress,and left ventricular mass index(LVMI)were compared between two groups after treatment 3 months and 6 months.Results After 3 months,BMI of the treatment group[(24.99±1.17)kg/m2]was significantly lower than that of the control group[(26.67±2.35)kg/m2,P<0.01].After 6 months,fasting insulin[(14.44±5.06)mU/L and(12.54±5.42)mU/L,P<0.05],insulin resistance index[(3.45±1.17)and(2.94±1.74),P<0.05],total cholesterol[(4.14±0.75)mmol/L and(3.95±0.64)mmol/L,P<0.05],systolic blood pressure and diastolic blood pressure,malonaldehyde and LVMI[(106.42±23.50)g/m2 and(97.87±22.68)g/m2,P<0.05]in the treatment group were lower than those in the control group.The level of superoxide dismutase in the treatment group was higher than that in the control group(P<0.05).Conclusion Huoxue Qianyang Qutan Recipe together with western medicine could reduce BMI in patients with obesity and hypertension,improve glycolipid metabolism,reduce blood pressure and the level of oxidative stress,and improve left ventricular hypertrophy.
