血清尿酸预测非瓣膜性心房颤动射频消融术后复发的相关性研究

摘　　要：目的探讨血清尿酸与非瓣膜性心房颤动(NVAF)病人射频消融术后复发的关系。方法收集2018年1月—2018年12月在河南省人民医院心内科首次行射频消融术的NVAF病人86例,按照复发与否分为复发组与未复发组,比较两组间临床资料,运用Logistic回归分析筛选出NVAF术后复发的独立预测因素。通过绘制心房颤动术后复发组危险因素的ROC曲线,判断预测因素准确性。结果共86例病人纳入研究,术后随访(8±4)个月,其中心房颤动复发27例,未复发59例。单因素分析表明:复发组病人术前尿酸、左心房前后径(LAD)、心房颤动病程、超敏C反应蛋白(hs-CRP)均高于未复发组,差异有统计学意义(P<0.05)。多元Logistics回归分析显示,LAD、尿酸和hs-CRP是NVAF射频消融术后复发的独立预测因素。结论血清尿酸是NVAF病人射频消融术后复发的一项独立预测因素。Objective To investigate serum uric acid in predicting the recurrence of nonvalvular atrial fibrillation after radiofrequency ablation.Methods Eighty-six patients with nonvalvular atrial fibrillation(NVAF)who underwent radiofrequency ablation for first time in from January 2018 to December 2018 were divided into recurrence group and non-recurrence group according to patients with or with out recurrence.The clinical data between two groups were compared,and the independent predictors of NVAF recurrence were screened by Logistic regression analysis.By drawing the ROC curve of risk factors in recurrence group,the accuracy of the predictors was determined.Results The follow-up period(8±4)months.Among them,27 patients with recurrent atrial fibrillation and 59 with out recurrence.Univariate analysis showed that preoperative uric acid,left atrium diameter(LAD),duration of atrial fibrillation,and hypersensitive C-reactive protein(hs-CRP)were higher in the recurrence group than that in the non-recurrence group(P<0.05).Multiple Logistic regression analysis showed that LAD,uric acid and hs-CRP were independent predictors of recurrence after NVAF radiofrequency ablation.Conclusion Serum uric acid was an independent predictor of recurrence after radiofrequency ablation in patients with nonvalvular atrial fibrillation.
