加味身痛逐瘀汤联合塞来昔布胶囊治疗多阶段脊髓型颈椎病术后轴性症状临床研究

摘　　要：目的:观察加味身痛逐瘀汤联合塞来昔布胶囊对多阶段脊髓型颈椎病术后轴性症状的疗效。方法:纳入多阶段脊髓型颈椎病采用后路单开门椎管扩大成形术治疗,且术后出现轴性症状的患者60例,随机分为对照组和治疗组各30例。对照组给予塞来昔布胶囊治疗,治疗组在对照组基础上给予加味身痛逐瘀汤治疗,2组均治疗4周。分别于治疗前、治疗4周采用颈椎功能障碍指数(NDI)评价2组颈椎功能,记录2组视觉模拟评分法(VAS)评分、颈椎关节活动度(ROM),比较2组疗效及不良反应情况。结果:治疗4周,2组VAS评分、NDI均降低(P<0.05),颈椎ROM均增大(P<0.05);治疗组VAS、NDI均低于对照组(P<0.05),颈椎ROM大于对照组(P<0.05)。治疗组总有效率为96.7%,对照组总有效率为83.3%,组间比较,差异有统计学意义(P<0.05)。治疗过程中,2组均未发生明显不良反应。结论:加味身痛逐瘀汤联合塞来昔布胶囊治疗多阶段脊髓型颈椎病术后轴性症状具有显著的临床疗效,可有效缓解患者颈部疼痛症状,减少颈椎功能障碍,改善颈椎功能。Objective:To observe the curative effect of modified Shentong Zhuyu tang combined with celecoxib capsules for axial symptoms after surgery of multilevel cervical myelopathy. Methods:A total of 60 cases of patients with multilevel cervical myelopathy who had axial symptoms after receiving unilaterally-open expansive laminoplasty through posterior approach,were randomly divided into the control group and the treatment group,30 cases in each group. The control group was treated with celecoxib capsules,and the treatment group was additionally given modified Shentong Zhuyu tang based on the treatment of the control group. The administration lasted for four weeks in both groups. Before treatment and after the four-week treatment,Neck Disabilitr Index(NDI) was used to evaluate the cervical function of the two groups,the scores of Visual Analogue Scale(VAS) and Range of motion(ROM) of cervical joint were recorded. The curative effect and the incidence of adverse reactions were compared between the two groups. Results:After four-week treatment,the scores of VAS and NDI were decreased in the two groups(P<0.05),and the cervical ROM were increased(P<0.05);the scores of VAS and NDI in the treatment group were lower than those in the control group(P<0.05),and the cervical ROM was higher(P<0.05). The total effective rate was 96.7% in the treatment group, and 83.3% in the control group, the difference being significant between the two groups(P<0.05). During treatment,there was no obvious adverse reactions in the two groups. Conclusion:For patients with axial symptoms after surgery of multilevel cervical myelopathy,the therapy of modified Shentong Zhuyu tang combined with celecoxib capsules has significant clinical effect,which can effectively relieve cervical pain,improve the function of cervical spine,and alleviate the dysfunction of cervical spine.
