摘 要:目的研究"四步法"医院感染防控全员培训考核模式,评价其应用效果,为其他医疗机构开展感染防控训练提供参考。方法 2019年6月25日-9月30日,选择江苏省人民医院(南京医科大学第一附属医院)全体医务人员5 425人(包括医生1 669人、护士3 129人、医技人员627人)作为培训考核对象。培训考核分为四个阶段:第一阶段(6.25~7.14)摸底考核;第二阶段(7.15~8.04)发布"应知应会"电子版手册;第三阶段(8.05~8.24)题库练习;第四阶段(8.25~9.14)再次考核,评估培训效果。结果第一阶段医务人员覆盖率为69.27%,第四阶段为102.47%,差异有统计学意义(P<0.001)。全部医务人员第四阶段平均得分(91.32±11.29)高于第一阶段(73.90±13.99),分数提高17.36分,差异有统计学意义(P<0.05)。第四阶段各类分层(性别、年龄段、职称)的医务人员平均得分均高于第一阶段(P<0.05)。8个部分的感控知识点习题考核正确率,第四阶段均高于第一阶段,差异有统计学意义(均P<0.001)。2个阶段的感控知识点分别纵向比较,第一和第四阶段的"消毒灭菌"和"传染病管理"平均得分均最低,差异均有统计学意义(P<0.05)。结论 "四步法"感控全员培训考核模式,结合培训与考核一体化,实践评价效果显著,实现了"全员覆盖"和"有效培训"。OBJECTIVE To study the "four-step method" sensory control training and assessment model for all employees and evaluate its application effect, so as to provide reference for other medical institutions to carry out sensory control training. METHODS From June 25, 2019 to September 30, 5425 medical staff(including 1, 669 doctors, 3129 nurses, 627 medical technicians) from Jiangsu People’s Hospital(the First Affiliated Hospital of Nanjing Medical University) were selected as the training and assessment subject. The training assessment was divided into four stages: the first stage(6.25~7.14) conducted a thorough assessment;the second stage(7.15 ~) issued the electronic version of the "required technical knowledge and skills" manual;the third stage(8.05~8.24) exercised in the question bank;the fourth stage(8.25~914) conducted another assessment to evaluate the training effect. RESULTS The coverage rate of medical personnel in the first stage was 69.27%, and that in the fourth stage was 102.47%, the difference was significant(P<0.001). The average score of all medical staff in the fourth stage(91.32±11.29) was significantly higher than that in the first stage(73.90±13.99), with an increase of 17.36 points(P<0.001). In the fourth stage, the average scores of medical personnel in various strata(gender, age group and professional title) were significantly higher than those in the first stage(P<0.05). The accuracy rate of exercises in the eight parts of sensory control knowledge point was significantly higher in the fourth stage than in the first stage(all P<0.001). In the longitudinal comparison of infection control knowledge points in the two stages, the average scores of "disinfection and sterilization" and "infectious disease management" in the first and fourth stages were the lowest(P<0.001).CONCLUSION The "four-step" mode of sensory control training and assessment for all staff, combined with the integration of training and assessment, had a significant effect on practical evaluation, achieving "full staff coverag
