摘 要:目的分析棉织布与长纤聚酯布手术铺巾在术中的细菌阻隔效果。方法入选2018年9月-2019年9月福建医科大学附属第二医院68例行百级层流手术间接受外科手术的患者,分为棉织布组与长纤聚酯布组,各34例,分析两种手术铺巾对细菌的阻隔作用。结果长纤聚酯布组患者男性占比52.94%,女性占比47.06%,其中骨性关节炎患者占比41.18%,类风湿关节炎占比47.06%,滑膜软骨瘤病占比11.76%与棉织布组比较差异无统计学意义;长纤聚酯布组患者年龄(55.12±1.97)岁,体质量(62.82±8.02)kg、手术时间(80.23±10.29)min、术中出血量(201.29±24.37)ml与棉织布组比较差异无统计学意义;长纤聚酯布组患者手术铺巾外层表面菌落培养阳性为8.82%、内层表面为2.94%,而棉织布组为29.41%、20.59%,差异有统计学意义(P<0.001);长纤聚酯布组平均静水压测试指标为(86.52±0.62)N/m2高于棉织布组(11.53±0.53)N/m2(P<0.001);长纤聚酯布组患者术后感染发生率为8.82%,棉织布组为20.59%,差异有统计学意义(P=0.019)。结论长纤聚酯布具有良好的细菌阻隔作用,针对骨科较为复杂,无菌需求较高的手术,长纤聚酯布具有更好的应用效果。OBJECTIVE To analyze the bacterial barrier effect of cotton cloth and long-fiber polyester surgical drape during operation. METHODS From September 2018 to September 2019, 68 patients who underwent surgery in the 100 level laminar flow operating room in the Second Affiliated Hospital of Fujian Medical University were selected and divided into the cotton cloth group and the long fiber polyester cloth group, with 34 cases in each group, and the barrier effect of two kinds of surgical towel on bacteria was analyzed. RESULTS In the experimental group, male accounted for 52.94%, female accounted for 47.06%, including osteoarthritis accounted for 41.18%, rheumatoid arthritis accounted for 47.06%, synovial chondromatosis accounted for 11.76%. There was no significantly difference between the two groups. The patients in the experimental group were(55.12±1.97) years old, body weight was(62.82±8.02) kg, operation time was(80.23±10.29) min, intraoperative blood loss was(201.29±24.37), which was no significant difference compared with the control group(P<0.001). The positive rates of colony culture on the outer surface and inner surface of the surgical drape in the experimental group were 8.82% and 2.94%, respectively, while those of the control group were 29.41% and 20.59%, respectively, the difference was statistically significant(P<0.001). The average hydrostatic pressure test index of the experimental group was(86.52 ± 0.62) n/m^2, which was higher than that of the control group(11.53±0.53) n/m^2(P<0.001). The incidence of postoperative infection was 8.82% in the experimental group and 20.59% in the control group(P=0.019). CONCLUSION Long-fiber polyester cloth had a good bacterial barrier effect, and it had a better application effect for more complicated orthopedic surgery and higher aseptic demand.
