摘 要:目的探究慢性阻塞性肺疾病(COPD)合并肺部感染患者转化生长因子-β1(TGF-β1)/Smads信号通路的表达。方法选取2015年1月-2018年12月南通大学附属医院收治的因肺部阴影行肺叶切除术患者150例,其中COPD患者100例,根据其诊断结果将合并肺部感染的COPD患者设置为感染组(52例),未合并感染的患者设置为未感染组(48例);将非COPD患者50例作为对照组。采用酶联免疫吸附法测定血清中白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)、TGF-β1水平。采用蛋白质免疫印迹法测定肺组织Smad2/3/6/7、TGF-β1蛋白的表达。结果肺部感染COPD患者致病菌以革兰阴性菌为主,多个菌种感染发生率高。感染组IL-8、TNF-α和TGF-β1水平,Smad2、Smad3和TGF-β1蛋白表达水平均高于未感染组和对照组,且未感染组的水平高于对照组,各组间比较差异具有统计学意义(P<0.05);感染组Smad6和Smad7蛋白表达水平低于未感染组和对照组,且未感染组的水平低于对照组,各组间比较差异具有统计学意义(P<0.05)。结论COPD患者发生肺部感染会导致各种炎性细胞合成并释放TGF-β1,从而使TGF-β1/Smads信号通路被激活,同时促进患者体内IL-8、TNF-α等炎性介质释放,TGF-β1/Smads信号转导途径中Smad2、Smad3发挥促进作用,而Smad6、Smad7则发挥抑制作用。OBJECTIVE To explore the expression of transforming growth factor-β1(TGF-β1)/Smads signaling pathway in chronic obstructive pulmonary disease(COPD)patients complicated with pulmonary infection.METHODS A total of 150 patients who underwent pulmonary lobectomy due to lung shadow in Affiliated Hospital of Nantong University from Jan 2015 to Dec 2018 were enrolled in the study,100 of who had COPD.According to the results of diagnosis,52 COPD patients who were complicated with pulmonary infection were assigned as the infection group,48 patients who were not complicated with infection were set as the non-infection group.Totally 50 patients who did not have COPD were chosen as the control group.The levels of serum interleukin-8(IL-8),tumor necrosis factor-α(TNF-α)and TGF-β1 were determined by using enzyme-linked immunosorbent assay.The expression of Smad2/3/6/7 and TGF-β1 protein in lung tissues were detected by means of Western blot method.RESULTS The gram-negative bacteria were dominant among the pathogens isolated from the COPD patients complicated with pulmonary infection.The incidence of infections caused by multiple species of pathogens was high.The levels of IL-8,TNF-α,TGF-β1,Smad2,Smad3 and TGF-β1 of the infection group were significantly higher than those of the non-infection group and the control group,the levels of above indexes of the non-infection group were significantly higher than those of the control group(P<0.05).The levels of Smad6 and Smad7 of the infection group were significantly lower than those of the non-infection group and the control group,the levels of above indexes of the non-infection group were significantly lower than those of the control group,and there were significant differences(P<0.05).CONCLUSION The pulmonary infection in the COPD patients can lead to the synthesis and release of TGF-β1 by various inflammatory cells,thereby activating TGF-β1/Smads signaling pathway and promoting the release of IL-8,TNF-αand other inflammatory mediators in the body of patients.Smad2 and S
