子痫前期患者胎盘组织中sFlt-1、PLGF及IGF-1的表达和临床意义

摘　　要：目的:探讨子痫前期(PE)孕妇胎盘组织中可溶性血管内皮生长因子受体-1(sFlt-1)、胎盘生长因子(PLGF)及胰岛素生长因子-1(IGF-1)的表达和临床意义。方法:收集临床诊断的重度PE孕妇25例和轻度PE15例分别作为重度PE组和轻度PE组,同期30例正常晚期妊娠孕妇作为对照组;取3组孕妇胎盘组织标本,采用免疫组织化学方法检测3组孕妇胎盘组织标本sFlt-1、PLGF及IGF-1的阳性表达水平。结果:轻度及重度PE组孕妇胎盘组织sFlt-1阳性表达高于对照组,且随病情加重而增加(P<0.05),轻度及重度PE孕妇胎盘组织中PLGF、IGF-1的阳性表达低于对照组,轻度PE组孕妇阳性表达低于重度PE组(P<0.05);轻度及重度PE组孕妇胎盘组织sFlt-1表达强度等级分别高于对照组(P<0.05),但PLGF和IGF-1的表达强度等级分别低于对照组(P<0.05),且轻度组孕妇胎盘组织PLGF和IGF-1的表达强度低于重度组孕妇(P<0.05)。结论:sFlt-1、PLGF及IGF-1的表达与PE的病情轻重有关,这3项指标可能共同参与了PE病情的发生与发展。Objective:To investigate the expression and clinical significance of soluble vascular endothelial growth factor receptor-1(sFlt-1),placental growth factor(PLGF)and insulin growth factor-1(IGF-1)in placenta of pregnant women with preeclampsia(PE).Methods:25 cases with severe PE and 15 cases with mild PE were collected as severe PE group and mild PE group respectively,and 30 normal late pregnant women were selected as the control group.Placental tissue samples from the three groups were taken,and the positive expression and intensity grade of sFlt-1,PLGF,and IGF-1 in placental tissue samples were detected by immunohistochemical method.Results:The positive expression of sFlt-1 in placental tissue of mild and severe PE groups was higher than that of the control group,and increased with the aggravation of the disease(P<0.05).The positive expression of PLGF and IGF-1 in placenta of mild and severe PE groups was lower than that of the control group,while it was lower in mild PE group than in severe PE group(P<0.05).The expression levels of sFlt-1 in placenta of mild and severe PE groups were higher than those of the control group(P<0.05).The expression intensity of PLGF and IGF-1 in placenta of mild and severe PE group were lower than that of severe PE group(P<0.05),while it was lower in mild PE group than in severe PE group(P<0.05).Conclusion:The expression of sFlt-1,PLGF,and IGF-1 is related to the severity of PE,and the three indexes are involved in the occurrence and development of PE.
