摘　　要：目的:分析采用阻抗心动描记法(ICG)评估不稳定心绞痛(UAP)患者接受经皮冠状动脉介入治疗(PCI)对血流动力学的影响。方法:选择2019年05月至2020年05月入我院诊断UAP患者共200例,其中50例接受保守药物治疗(对照组),另150例接受PCI治疗(观察组)。采用ICG评估治疗前、治疗后24h和7d的血流动力学变化,包括心率(HR)、呼吸频率(RR)、心排量/心指数(CO/CI)、搏出量/搏出指数(SV/SI)、全身外周阻抗/外周阻抗指数(TPR/TPRI)、左心做功指数(LCWI)。结果:两组患者治疗前的一般临床资料比较,差异无统计学意义(P>0.05)。两组治疗后CO/CI、SV/SI和LCWI较治疗前升高,TPR/TPRI降低,且观察组比对照组改善更加明显(P<0.05)。结论:采用ICG检测UAP患者PCI治疗对血流动力学影响有助于评估手术的安全性和临床疗效,值得推广应用。Objective The study aimed to explore the effect of hemodynamics in patients with unstable angina pectoris(UAP)underwent percutaneous coronary intervention(PCI)treatment by impedance cardiography(ICG).Method A total of 200 UAP patients from May 2019 to May 2020 were chosed,50 patients of which received conservative drugs therapy as control group and other 150 patients adopted PCI as observation group.Then to compare differences of hemodynamics indicators including heart rate(HR),respiratory rate(RR),cardiac output/heart index(CO/CI),stroke volume/stroke index(SV/SI),systemic peripheral impedance/peripheral impedance index(TPR/TPRI),left heart work index(LCWI)before treatment,after treatment 24h and 7d with ICG.Results The common clinical datas before treatment in the two groups were no differences(P>0.05).The CO/CI,SV/SI and LCWI values after treatment 24h and 7d in the two groups were gradually increasing,TPR/TPRI lowering,what’s more,the changes in observation group were all significantly more than control group(P<0.05).Conclusion ICG can greatly detect hemodynamics changes for UAP patients with PCI,which is helpful to evaluate the safety and clinical effect of the operation and is worth popularizing.
