摘 要:目的:探讨红细胞分布宽度(RDW)、血红蛋白(Hb)水平在预测慢性阻塞性肺疾病(COPD)急性加重及判定COPD严重程度中的应用价值。方法:对石河子大学医学院第一附属医院接诊的121例COPD患者(因病情急性加重入院,治疗后其病情进入稳定期)的临床资料进行回顾性分析。分别于这些患者入院后24 h内和其病情进入稳定期后对其进行RDW、Hb检测,然后比较入院后24 h内与病情进入稳定期后其RDW、Hb的水平。在这些患者的病情进入稳定期后,对其进行肺功能检查,并比较其中不同肺功能分级患者RDW、Hb的水平。采用多因素logistic回归分析法分析这些患者的RDW、Hb的水平是否是其病情加重的影响因素。结果:1)与病情进入稳定期后相比,入院后24 h内这些患者的RDW较高,其Hb的水平较低,P<0.05。2)与肺功能分级为Ⅰ级的患者相比,这些患者中肺功能分级为Ⅱ级患者的RDW较高,其Hb的水平较低,P<0.05。与肺功能分级为Ⅱ级的患者相比,这些患者中肺功能分级为Ⅲ级患者的RDW较高,其Hb的水平较低,P<0.05。与肺功能分级为Ⅲ级的患者相比,这些患者中肺功能分级为Ⅳ级患者的RDW较高,其Hb的水平较低,P<0.05。3)进行多因素logistic回归分析的结果显示,这些患者RDW、Hb的水平均为其病情加重的影响因素。结论:RDW、Hb水平在预测COPD急性加重及判定COPD严重程度中的应用价值较高。Objective:To investigate the value of erythrocyte distribution width(RDW)and hemoglobin(Hb)level in predicting the acute exacerbation of chronic obstructive pulmonary disease(COPD)and determining the severity of COPD.Methods:The clinical data of 121 patients with COPD(admitted to the first Affiliated Hospital of Shihezi University Medical College due to acute exacerbation of their condition,and their condition entered a stable period after treatment)were retrospectively analyzed.RDW and Hb tests were performed on these patients within 24 h after admission and after their condition entered the stable period,and then their RDW and Hb levels were compared with those within 24 h after admission.After the condition of these patients entered the stable period,lung function examination was conducted,and the levels of RDW and Hb in patients with different grades of lung function were compared.Multivariate logistic regression analysis was used to analyze whether the levels of RDW and Hb in these patients were the influencing factors for the aggravation of their illness.Results:1)Compared with the stable period,these patients had higher RDW and lower Hb level within 24 h after admission(P<0.05).2)compared with patients with lung function class forⅠ,these patients class forⅡRDW was higher in patients with lung function,the Hb level is low,P<0.05).Compared with patients with lung function class forⅡ,these patients class forⅢRDW was higher in patients with lung function,the Hb level is low,P<0.05).Compared with patients with lung function class forⅢ,these patients class forⅣRDW was higher in patients with lung function,the Hb level is low,P<0.05).3)The results of multivariate logistic regression analysis showed that the levels of RDW and Hb in these patients were the influencing factors for the aggravation of their illness.Conclusion:RDW and Hb levels have higher application value in predicting the acute exacerbation of COPD and determining the severity of COPD.
