摘　　要：目的:探讨并研究用经剑突下切口电视胸腔镜手术(S-VATS)治疗前纵隔肿瘤的效果。方法:选取2017年1月至2019年8月期间湖南省人民医院收治的66例前纵隔肿瘤患者作为研究对象。随机将其分为S-VATS组(n=34)与I-VATS组(n=32)。分别采用S-VATS和经肋间侧切口电视胸腔镜手术(I-VATS)对S-VATS组患者与I-VATS组患者进行治疗,然后比较两组患者各项手术指标。结果:S-VATS组患者麻醉诱导至完成气管插管的时间短于I-VATS组患者,术后48 h其视觉模拟评分法(VAS)的评分低于I-VATS组患者,差异有统计学意义(P<0.05)。S-VATS组患者术后咽部不适和肩关节活动障碍的发生率均低于I-VATS组患者,差异有统计学意义(P<0.05)。结论:用S-VATS治疗前纵隔肿瘤能缩短患者气管插管的时间,减轻其术后的疼痛感,降低其术后咽部不适和肩关节活动障碍的发生率。Objective:To investigate and study the effect of video-assisted thoracoscopic surgery(S-VATS)through subxiphoid incision in the treatment of anterior mediastinal tumor.Methods:66 patients with anterior mediastinal tumor admitted to Hunan Provincial People’s Hospital from January 2017 to August 2019 were selected as study subjects.They were randomly divided into S-VATS group(n=34)and I-VATS group(n=32).S-vats and I-VATS were used to treat patients in s-VATS group and I-VATS group respectively,and then various surgical indicators were compared between the two groups.Results:The time from anesthesia induction to endotracheal intubation in s-VATS group was shorter than that in I-VATS group,and the visual analogue scale(VAS)score 48 h after operation was lower than that in I-VATS group,with statistically significant difference(P<0.05).The incidence of postoperative pharyngeal discomfort and shoulder joint activity disorder in s-VATS group was lower than that in I-VATS group,and the difference was statistically significant(P<0.05).Conclusion:S-VATS treatment for anterior mediastinal tumor can shorten the time of endotracheal intubation,reduce postoperative pain,and reduce the incidence of postoperative pharyngeal discomfort and shoulder joint movement disorder.
