自拟益气养阴化瘀汤治疗冠心病稳定型心绞痛(气阴两虚兼瘀证)的临床观察

摘　　要：目的观察自拟益气养阴化瘀汤治疗冠心病稳定型心绞痛(气阴两虚兼瘀证)的临床疗效。方法选取60例冠心病稳定型心绞痛(气阴两虚兼瘀证)患者,随机分为两组,对照组参照指南予抗血小板聚集、扩张冠脉等常规治疗,治疗组在对照组基础上予以自拟益气养阴化瘀汤口服治疗,均服药2周。观察两组心电图疗效、心绞痛改善情况、主要症状积分、硝酸甘油减停率、内皮功能指标及B型尿钠肽(BNP)水平。结果治疗组心电图改善总有效率优于对照组,差异无统计学意义(P>0.05);治疗组心绞痛改善总有效率优于对照组(P<0.05);治疗组治疗后胸闷、胸痛、气短、心悸症状评分明显低于对照组(P<0.05);治疗组硝酸甘油减停率优于对照组(P<0.05);治疗组治疗后内皮功能指标、BNP水平优于对照组(P<0.05)。结论采用自拟益气养阴化瘀汤治疗冠心病稳定型心绞痛(气阴两虚兼瘀证),能够有效缓解患者的胸闷、胸痛等临床症状,减少硝酸甘油用量,改善内皮功能,不良反应小。Objective:To observe the clinical effect of self-made Yiqi Yangyin Huayu Decoction in the treat⁃ment of stable angina pectoris of coronary heart disease(qi and yin deficiency and stasis syndrome).Methods:A total of 60 patients with stable angina pectoris(qi and yin deficiency and stasis syndrome)with coronary heart dis⁃ease were selected as the research subjects,and were randomly divided into a treatment group(n=30)and a con⁃trol group(n=30).The control group were used the conventional treatments such as anti-platelet aggregation and coronary dilation,the treatment group were given oral treatment of self-made Yiqi Yangyin Huayu Decoction on the basis of the control group,and all took medication for 2 weeks.After treatment,the effect of electrocardiogram,im⁃provement of angina pectoris,score of main symptoms,nitroglycerin reduction rate,endothelial function index and B-type urinary natriuretic peptide(BNP)level were observed.Results:The total effective rate of ECG improve⁃ment in the treatment group was better than that of the control group,and the difference was not statistically signifi⁃cant(P>0.05);the total effective rate of angina improvement in the treatment group was better than that of the control group(P<0.05);chest tightness,chest pain,shortness of breath and the palpitations symptom score in the treatment group significantly lower than those in the control group(P<0.05);the nitroglycerin reduction rate of the treatment group was better than that in the control group(P<0.05);the endothelial function indexes and BNP lev⁃els of the treatment group after treatment were better than those in the control group(P<0.05).Conclusion:Selfmade Yiqi Yangyin Huayu Decoction for the treatment of stable angina pectoris of coronary heart disease(qi and yin deficiency and stasis syndrome)can effectively relieve the patients′clinical symptoms such as chest tightness and chest pain,reduce the amount of nitroglycerin,improve endothelial function.Its adverse reactions were less.
