摘　　要：目的探讨不同前列腺特异性抗原(PSA)水平对超声造影检出前列腺癌的价值以及影响因素。方法选取2016年1月至2018年6月在徐州市中医院进行超声造影诊断为疑似前列腺癌的157例患者作为研究对象。按照不同的PSA水平分为三组,收集超声造影以及病理组织活检结果,同时对超声造影检出前列腺癌的影响因素进行分析。结果病理活检结果确诊前列腺癌104例,非前列腺癌53例。超声造影结果显示,前列腺癌在达峰时间和显影时间低于非前列腺癌,而前列腺癌在增强强度明显高于非前列腺癌,差异具有统计学意义(P<0.05)。单因素及多因素分析结果显示,PSA水平及前列腺癌分期是影响超声造影检出前列腺癌的独立影响因素,差异具有统计学意义(P<0.05)。结论超声造影检出前列腺癌具有较高价值,尤其是对于高PSA水平的患者,PSA水平及前列腺癌分期是影响超声造影检出前列腺癌的独立影响因素。Objective To explore the value of different levels of prostate-specific antigen(PSA)in the detection of prostate cancer by contrast-enhanced ultrasound and its influencing factors.Methods A total of 157 patients with suspected prostate cancer diagnosed by contrast-enhanced ultrasound in Xuzhou Hospital of Traditional Chinese Medicine from January 2016 to June 2018 were selected as study subjects,and were divided into three groups according to different PSA levels.contrast-enhanced ultrasound and pathological biopsy results were collected,and the influencing factors of contrast-enhanced ultrasound detection of prostate cancer were analyzed.Results Pathological biopsy results confirmed prostate cancer in 104 cases and non-prostate cancer in 53 cases.contrast-enhanced ultrasound showed that the peak-reaching time and development time of prostate cancer were lower than those of non-prostate cancer,while the enhancement intensity of prostate cancer was significantly higher than that of non-prostate cancer(P<0.05).Univariate and multivariate analysis showed that PSA level and prostate cancer stage were independent influencing factors for contrast-enhanced ultrasound detection of prostate cancer,and the difference was statistically significant(P<0.05).Conclusion contrast-enhanced ultrasound has a high value for detecting prostate cancer,especially for patients with high PSA levels.PSA levels and prostate cancer staging are independent factors influencing the detection of prostate cancer by contrast-enhanced ultrasound.
