妇科恶性肿瘤患者腹腔镜术后应用空气压力波治疗仪联合低分子肝素钙注射液预防下肢深静脉血栓形成的疗效观察

摘　　要：目的探讨妇科恶性肿瘤患者腹腔镜术后应用空气压力波治疗仪联合低分子肝素钙注射液预防下肢深静脉血栓形成(LEDVT)的临床疗效。方法选取2017年6月至2019年1月秦皇岛市第一医院诊治的130例拟行腹腔镜手术治疗的妇科恶性肿瘤患者作为研究对象。按照入院时间顺序编号,采用随机数字表法分为观察组(n=65)和对照组(n=65)。对照组患者术后1d给予低分子肝素钙注射液治疗,观察组患者在对照组基础上联合应用空气压力波治疗仪治疗。比较两组患者术前及术后第5d凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、D-二聚体(D-D)、血浆纤维蛋白原(FIB)、血小板(PLT)、双下肢股静脉血流速度等指标,并统计LEDVT及肺栓塞发生率。结果术前,两组患者PLT、APTT、PT、FIB以及D-D比较,差异无统计学意义(P>0.05);术后第5d,观察组患者APTT、PT、FIB以及D-D与对照组患者比较,差异具有统计学意义(P<0.05)。两组患者术后双下肢股静脉(右、左)血流速度比较,差异具有统计学意义(P<0.05)。对照组患者LEDVT发生率为7.77%(5/65),观察组患者LEDVT发生率为1.53%(1/65),两组比较,差异无统计学意义(P>0.05)。两组患者均未出现术后肺栓塞。结论妇科恶性肿瘤患者腹腔镜术后应用空气压力波治疗仪联合低分子肝素钙注射液治疗,可改善凝血功能,降低术后D-D,改善血流状态,降低LEDVT发生率,且安全性高,值得推广应用。Objective To investigate the clinical efficacy of air pressure combined with low molecular heparin calcium injection on prevention of lower extremity deep venous thrombosis(LEDVT)in patients with gynecological malignant tumor after laparoscopic surgery Methods 130 patients with gynecological malignant tumor undergoing laparoscopic surgery in the First Hospital of Qinhuangdao from June 2017 to January 2019 were selected,whom were divided into observation group(n=65)and control group(n=65)by random number table method.Patients in the control group were treated with low molecular heparin calcium injection one day after operation,and patients in the observation group were treated with air pressure combined with low molecular heparin calcium injection.Prothrombin time(PT),activated partial thrombin time(APTT),D-dimer(D-D),plasma fibrinogen(FIB),platelet(PLT),and bilateral lower limb femoral vein blood flow velocity were compared between the two groups before and on the 5 th day after surgery,and LEDVT and pulmonary embolism incidence were calculated.Results There was no significant difference in PLT,APTT,PT,FIB and D-D between the two groups before surgery(P>0.05).On the 5 th day after the operation,APTT,PT,FIB and D-D of the observation group were compared with those of the control group,and the differences were statistically significant(P<0.05).The blood flow velocity of the femoral vein(right and left)of both lower limbs after operation was compared between the two groups,and the difference was statistically significant(P<0.05).The incidence of LEDVT in the control group was 7.77%(5/65),while that in the observation group was 1.53%(1/65),there was no statistically significant difference between the two groups(P>0.05).There was no postoperative pulmonary embolism in either group.Conclusions The application of air pressure combined with low molecular heparin calcium injection after laparoscopic surgery in patients with gynecological malignant tumors can improve the coagulation function,reduce postoperative D-D,improv
