卵巢浆液性癌中eIF3a的表达及与血清中HE4的相关性

摘　　要：目的探讨卵巢浆液性癌中真核翻译起始因子3a(eIF3a)表达的临床价值,分析其与细胞增殖、转移和人附睾分泌蛋白4(HE4)表达的关系。方法选取2013年1月至2014年5月保定市第一中心医院经病理确诊的71例卵巢浆液性癌患者(研究组)、71例卵巢交界性浆液性囊腺瘤患者(对照组1)和71例卵巢浆液性囊腺瘤患者(对照组2)作为研究对象。应用免疫组化方法检测三组患者中eIF3a的表达,检测研究组患者中增殖基因Ki67和基质金属蛋白酶-2(MMP-2)的表达。应用Western Blot法检测研究组患者中上皮型黏附蛋白(E-cadherin)和基质金属蛋白酶-9(MMP-9)的表达。应用化学发光法检测研究组患者术前血清中HE4的表达。结果三组患者中eIF3a的表达比较,差异具有统计学意义(P<0.05);研究组患者中eIF3a表达的阳性率在不同肿瘤最大径、是否双侧发生、脉管浸润、病变分级、淋巴结转移和FIGO分期中的表达比较,差异具有统计学意义(P<0.05)。EIF3a表达的阳性率与生存时间有关(P<0.05)。相关分析显示eIF3a与Ki67(r=0.50,P<0.05)、eIF3a与MMP-2(r=0.59,P<0.05)、eIF3a与MMP-9(r=0.51,P<0.05)、eIF3a与血清中HE4(r=0.54,P<0.05)均呈正相关性,eIF3a与E-cadherin呈负相关性(r=-0.59,P<0.05)。结论EIF3a在卵巢浆液性癌中表达上调,可以促进肿瘤的形成和进展,EIF3a与肿瘤细胞增殖和转移指标相关。EIF3a与患者血清中HE4的表达具有相关性。EIF3a高表达时预示着肿瘤不良的生物学行为和预后。Objective To investigate the clinical value of eukaryotic translation initiation factor 3 a(eIF3 a)expression in ovarian serous carcinoma,and to analyze its relationship with cell proliferation,metastasis and human epididymal protein 4(HE4)expression.Methods 71 cases of ovarian serous carcinoma pathological diagnosed in Baoding First Central Hospital from January 2013 and May 2014,were selected as the study group,ovarian borderline serous cystadenoma as control group 1,71 cases of ovarian serous cystadenoma as control group 2.The expression of eIF3 a in the three groups was detected by immunohistochemistry,and the expression of the proliferative gene Ki67 and matrix metalloproteinase-2(MMP-2)in the study group was detected.Western Blot was used to detect the expression of E-cadherin and MMP-9.HE4 expression in serum was detected by chemiluminescence.Results The expression of eIF3 a in the three groups was significantly different(P<0.05).In the study group,the positive rate of eIF3 a expression was significantly different in the maximum tumor diameter,bilateral occurrence,vascular infiltration,lesion grade,lymph node metastasis and FIGO stage(P<0.05).The positive rate of EIF3 a expression was related to survival time(P<0.05).Correlation analysis showed that eIF3 a was positively correlated with Ki67(r=0.50,P<0.05),eIF3 a with MMP-2(r=0.59,P<0.05),eIF3 a with MMP-9(r=0.51,P<0.05),eIF3 a with HE4(r=0.54,P<0.05),while eIF3 a was negatively correlated with E-cadherin(r=-0.59,P<0.05).Conclusions The up-regulated expression of EIF3 a in ovarian serous carcinoma can promote tumor formation and progression,and EIF3 a is related to tumor cell proliferation and metastasis.EIF3 a was correlated with serum HE4 expression.High expression of EIF3 a predicts poor biological behavior and prognosis.
