改良阴式子宫切除术应用于老年子宫脱垂患者对其并发症发生率及生活质量的影响

摘　　要：目的探讨改良阴式子宫切除术应用于老年子宫脱垂患者对其并发症发生率及生活质量的影响。方法选取2017年3月至2018年3月入住沈阳市第四人民医院的82例老年子宫脱垂患者作为研究对象。按治疗方式分为研究组(n=41)和对照组(n=41),研究组予以改良阴式子宫切除术,对照组仅予以常规阴式子宫切除术,比较两组患者治疗有效率、手术情况、住院时间、并发症发生率、心理状态变化情况及生存质量变化情况。结果对照组患者治疗有效率低于研究组患者,差异具有统计学意义(P<0.05);对照组患者手术时间及住院时间长于研究组患者,术中出血量多于研究组患者,差异具有统计学意义(P<0.05);对照组患者并发症发生率高于研究组患者,差异具有统计学意义(P<0.05);治疗后对照组患者SAS、SDS评分均高于研究组患者,差异具有统计学意义(P<0.05);治疗后对照组患者生存质量低于研究组患者,差异具有统计学意义(P<0.05)。结论改良阴式子宫切除术治疗老年子宫脱垂患者的临床效果显著,安全性较高,值得在临床推广与应用。Objective To investigate the effect of modified vaginal hysterectomy on the incidence of complications and quality of life in elderly patients with uterine prolapse.Methods A total of 82 elderly patients with uterine prolapse in Shenyang Fourth People′s Hospital from March 2017 to March 2018 were enrolled,and divided into research group(n=41)and the control group(n=41)according to the treatment.The study group was treated with modified vaginal hysterectomy,while the control group was treated only with routine vaginal hysterectomy.Treatment efficient,operation situation,hospital stay,complication rate,psychological state and the change of the quality of life were compared between the two groups.Results The effective rate of patients in the control group was lower than that in the study group,and the difference was statistically significant(P<0.05).The operative time and hospital stay of the control group were longer than those of the study group,and the intraoperative blood loss was higher than that of the study group,with statistically significant difference(P<0.05).The complication rate in the control group was higher than that in the study group,and the difference was statistically significant(P<0.05).After treatment,SAS and SDS scores of patients in the control group were higher than those in the study group,and the difference was statistically significant(P<0.05).The quality of life in the control group was lower than that in the study group,and the difference was statistically significant(P<0.05).Conclusions Modified vaginal hysterectomy for elderly patients with uterine prolapse has a significant clinical effect,high safety,worthy of clinical promotion and application.
