帕金森病患者临床特征与生活质量关系的调查研究

摘　　要：目的探讨抑郁、焦虑、睡眠障碍、帕金森病症状、帕金森病用药情况与健康相关生活质量之间的关系,并确定帕金森病患者健康相关生活质量的预测因素。方法采用简易智能精神状态检查量表筛查,并对评分大于或等于24分的134例帕金森病患者进行一系列的问卷和量表评定,其中包括统一帕金森病评定量表、39项帕金森病生存质量调查表、帕金森病睡眠量表、贝克抑郁自评量表及贝克焦虑自评量表。结果自我评价生活质量不佳的患者病程更长,帕金森病症状更严重,Hoehn-Yahrp评级更高,左旋多巴用药剂量更高,焦虑和抑郁水平更高,睡眠障碍更严重,整体认知状态更差,差异均有统计学意义(P<0.05)。结论认知状态、日常生活活动依赖程度、抑郁和焦虑是帕金森病患者生活质量的预测因素,且是帕金森病患者生活质量不佳的重要独立因素。Objective To investigate the relationship between depression,anxiety,sleep disorders,Parkinson′s disease(PD)symptoms and PD medication with the health related quality of life(QOL),and to determine the predictive factors of health related quality of life in PD patients.Methods The Simple Mental State of intelligence Checklist was adopted to screen out the patients.Then 134 PD patients with the score≥24 points further conducted a series of questionnaire and scales evaluation,including the Unified Parkinson′s Disease Rating Scale,39 Quality of Life for Parkinson′s Disease Scale,Parkinson′s Disease SleePScale,Beck Depression Inventory and Beck Anxiety Inventory.Results The patients with poor quality of life in self evaluation had longer disease course,more severe PD symptoms,higher Hoehn Yahr grade,higher levodopa dose,higher anxiety and depression levels,more severe sleep disorder and poorer overall cognitive status,and the differences were statistically significant(P<0.05).Conclusion The cognitive status,dependence degree of daily life activities,depression and anxiety are the predictive factors for the quality of life in PD patients,moreover which are the important independent factors of poor quality of life in PD patients.
