摘 要:目的比较彩色多普勒超声(CDU)与血清甲胎蛋白(AFP)检测在原发性肝癌(PHC)临床诊断中的应用价值。方法选取2019年1~12月于医院就诊的40例PHC患者为A组,选取同一时期在医院就诊的40例非PHC的肝脏占位性病变患者为B组,对上述研究对象均分别采用CDU与血清AFP检测,分析比较两种方法的诊断效能和联合诊断的价值。结果采用CDU检测时,A组患者确诊率为77.50%,漏诊率22.50%,DPI和RR明显高于B组,但PCI明显低于B组(P <0.05)。采用血清AFP检测时,A组患者确诊率为52.50%,漏诊率47.50%。采用CDU方法诊断PHC的灵敏度、特异性和准确度分别为77.50%、90.00%和83.75%;采用血清AFP检测诊断PHC的灵敏度、特异性和准确度分别为52.50%、85.00%和68.75%,CDU和血清AFP方法的检测特异性无明显差异(P> 0.05),但是CDU方法的检测灵敏度(P <0.05)和准确度(P <0.05)均明显高于血清AFP检测方法。当CDU与AFP检测联用时,两者联合检测的灵敏度为97.50%,明显高于单一使用CDU检测(77.50%)(P <0.05)或血清AFP检测(52.50%)(P <0.05)。两者联合检测的特异性为87.50%,与单一使用CDU(90.00%)或血清AFP检测方法(85.00%)的特异性相比无统计学差异(P> 0.05)。结论 CDU方法检测PHC的临床价值比血清AFP检测更大,两者联合检测时可诊断95%以上的PHC患者,对于PHC的临床诊断和鉴别诊断具有重要的参考价值。Objective To compare the application value of color Doppler ultrasound(CDU) method and serum alpha-fetoprotein(AFP) test in the clinical diagnosis of primary hepatic cancer(PHC). Methods A total of 40 patients with PHC who visited our hospital from January 2019 to December 2019 were selected and included in group A, 40 patients with a space-occupying lesion in the liver and without PHC who visited our hospital during the same period were selected and included in group B. CDU method and AFP test were performed for the above study subjects, respectively. An analysis and comparison were made for the two methods in terms of their diagnostic efficiency and the value of combined diagnosis. Results When using the CDU method, the rate of correct diagnosis and the rate of missed diagnosis in group A were 77.50% and 22.50%, respectively. Group A had markedly higher doppler perfusion index(DPI) and resistance ratio(RR) than group B, and an obviously lower portal congestion index(PCI), with both differences showing statistical significance(P < 0.05). When performing an AFP test, the rate of correct diagnosis and the rate of missed diagnosis in group A were 52.50% and 47.50%. The sensitivity, specificity, and accuracy of the CDU method in the diagnosis of PHC were 77.50%, 90.00%, and 83.75%, respectively;the sensitivity, specificity, and accuracy of AFP test in the diagnosis of PHC were 52.50%, 85.00%, and 68.75%, respectively. CDU method and serum AFP test showed no marked difference in the detection specificity(P > 0.05). However, CDU method showed higher detection sensitivity(P < 0.05) and accuracy(P < 0.05) than serum AFP test. When the CDU method was used in combination with the AFP test, the sensitivity was 97.50%, significantly higher than that when using the CDU method alone(77.50%)(P < 0.05) or using the serum AFP test alone(52.50%)(P < 0.05).
