摘　　要：目的:分析重庆医科大学附属第二医院近3年第二类精神药品消耗情况和用药合理性,为临床使用和管理提供参考。方法:HIS系统调取2016年至2018年第二类精神药品的消耗情况,统计分析用药频度(defined daily doses,DDDs)、日均费用(defined daily dose consumption,DDDc)和排序比(B/A)等指标。结果:本院第二类精神药品占全院药品总金额比例小于1%,但近3年占比和增幅均逐年加大。2016年至2018年消耗金额排名前5的是地佐辛注射液、咪达唑仑注射液、右佐匹克隆片、盐酸曲马多注射液和盐酸曲马多缓释片(0.1g);DDDs排序前5的是艾司唑仑片、阿普唑仑片、地佐辛注射液、右佐匹克隆片和氯硝西泮片;DDDc排名前5的是喷他佐辛注射液、地佐辛注射液、丁丙诺啡透皮贴剂、咪达唑仑注射液和盐酸曲马多注射液;销售金额前10的药品中,地佐辛注射液、右佐匹克隆片、咪达唑仑注射液、盐酸曲马多注射液、盐酸曲马多缓释片(0.1 g、0.15 g)和丁丙诺啡透皮贴剂的B/A排序比小于1。结论:我院2016年至2018年第二类精神药品消耗量显著增加,使用合理性较高,但少数消耗金额占比较大、价格较高的品种需加强应用监管;部分新药物和新剂型应用较少,需加强理论学习、原因分析和采取措施。Objective:To analyze the volume and rationality of prescribing psychotropic drugs of category Ⅱ in our hospital from 2016 to 2018,and to provide a reference for their clinical use and supervision.Methods:The consumption of psychotropic drugs of category Ⅱ from 2016 to 2018 was collected from the hospital information system(HIS)for statistical analysis of defined daily doses(DDDs),defined daily dose consumption(DDDc),and ranking ratio(B/A).Results:Psychotropic drugs of category Ⅱ accounted for less than 1% of the total drug sales in our hospital,but the percentage increased and the increase accelerated from 2016 to 2018.In these years,the top five psychotropic drugs of category Ⅱ in terms of sales were dezocine injection,midazolam injection,eszopiclone tablet,tramadol hydrochloride injection,and tramadol hydrochloride sustained-release tablet(0.1 g).The top five drugs in terms of DDDs were estazolam tablet,alprazolam tablet,dezocine injection,eszopiclone tablet,and clonazepam tablet.The top five drugs in terms of DDDc were pentazocine injection,dezocine injection,buprenorphine transdermal patch,midazolam injection,and tramadol hydrochloride injection.Among the top 10 drugs with the highest sales,B/A values of less than 1 were found for dezocine injection,eszopiclone tablet,midazolam injection,tramadol hydrochloride injection,tramadol hydrochloride sustained-release tablet(0.1 g,0.15 g),and buprenorphine transdermal patch.Conclusion:The consumption of psychotropic drugs of category Ⅱ increased markedly in our hospital from 2016 to 2018,with rational use in most cases.However,some psychotropic drugs of category Ⅱ with high DDDs and DDDc should be kept under strict supervision.Some new drugs and new dosage forms were less used,which requires study of related theories,analysis of causes,and implementation of countermeasures.
