早发性癫痫脑病患儿脑电图与脑功能预后的相关性

摘　　要：目的探究早发性癫痫脑病(early-onset epileptic encephalopathies,EOEE)患儿脑电图与脑功能预后的相关性。方法回顾性分析156例EOEE患儿临床资料,进行脑电图检查并分级,采用大脑整体表现量表(pediatric cerebral and overall performance category scale,PCOPCS)评估患儿脑功能预后情况,采用Pearson分析脑电图特点与脑功能预后的相关性。结果脑电图正常者24例,脑电图异常者132例,主要表现是α节律不明显,背景节律减慢,双侧颞区及额区少量的中尖波、尖慢波、尖化θ/δ波,波幅及频率均呈不规则,尖化波、棘慢波、尖慢波在颞区、额区发放,右侧颞区最多,其中轻度异常42例,中度异常53例,重度异常37例。预后良好82例,预后不良37例,预后严重不良37例。随着脑电图异常严重程度增加,脑功能预后评分升高,差异均有统计学意义(P<0.05)。经Pearson分析显示,脑电图分级与EOEE患儿脑功能预后评分呈正相关(r=0.864,P=0.002)。结论脑电图能够反映EOEE患儿病情严重程度,且分级越高,患儿脑功能预后越差。Objective To explore the electroencephalogram(EEG)of children with early-onset epileptic encephalopathies(EOEE)and its correlation with the prognosis of brain function.Methods The clinical data of 156 children with EOEE were retrospectively analyzed,EEG was performed and graded,and the prognosis of the brain function of the children was assessed using the pediatric cerebral and overall performance category scale(PCOPCS),and EEG was analyzed the correlation between characteristics and brain function prognosis by Pearson.Results There were 24 cases with normal EEG and 132 cases with abnormal EEG.The main manifestations were thatαrhythm was not obvious,and background rhythm was slowed down;a small amount of middle sharp wave,sharp slow wave and sharpθ/δwave in bilateral temporal area and frontal area were irregular in amplitude and frequency;sharp slow wave,spinous slow wave and sharp slow wave were distributed in temporal area and frontal area,and most of them were in right temporal area,of which 42 cases were slight abnormal,53 cases were moderate abnormal,and 37 cases were severe abnormal.The prognosis was good in 82 cases,poor in 37 cases and serious in 37 cases.With the increase of the severity of EEG abnormality,the prognosis score of brain function increased,and the difference was statistically significant(P<0.05).Pearson analysis showed that the EEG grade was positively correlated with the prognosis score of brain function of EOEE(r=0.864,P=0.002).Conclusion EEG can reflect the severity of EOEE,and the higher the grade,the worse the prognosis of brain function.
