摘　　要：目的探讨医院儿科特殊使用类抗菌药物的应用情况,为规范儿科抗菌药物合理应用提供参考。方法回顾性分析2018年1月1日~2019年12月13日儿科应用特殊使用级抗菌药物的637例患儿病例,汇总其在不同科室的应用比例,不同种类药物的使用情况,患儿疾病类型分布,细菌分布特点以及标本构成等。结果 2018~2019年收治的637例应用特殊使用类抗菌药物患儿中,分布最多的科室为NICU 242例(37.99%)、PICU 130例(20.41%);使用频率最高的品类为亚胺培南242例(37.99%)、美罗培南194例(30.46%)、利奈唑胺93例(14.60%);比例最高的疾病为早产儿65例(10.20%)、呼吸窘迫综合征63例(9.89%)、肺炎55例(8.63%)。结论儿科应用特殊使用类抗菌药物的规范性较好,但仍存在一定比例不合理用药情况,临床还需严格把控特殊使用类抗菌药物的适应证,并加强细菌培养与耐药性监测,汇总不同阶段抗菌药物的应用特点,为抗菌药物合理、规范应用提供参考。Objective To explore the application of special antibacterial drugs in pediatric hospitals,and to provide a reference for regulating the rational application of antibacterial drugs in pediatrics.Methods A retrospective analysis of 637 cases of pediatric patients using special-use antibacterial drugs from January 1,2018 to December 13,2019,and summarizing their application rates in different departments,the use of different types of drugs,children disease type distribution,bacterial distribution characteristics and specimen composition,etc.Results Among the 637 children treated with special antibacterial drugs in our hospital from 2018 to 2019,the most distributed departments were NICU242(37.99%)and PICU130(20.41%);the most frequently used category was imipenem 242 cases(37.99%),meropenem 194 cases(30.46%),linezolid 93 cases(14.60%);the highest proportion of diseases were 65 cases of premature infants(10.20%),63 cases of respiratory distress syndrome(9.89%),55 cases of pneumonia(8.63%).Conclusion The pediatric application of our hospital uses special antibacterial drugs with good standardization,but there are still a certain proportion of irrational medications.The clinical need to strictly control the indications of special antibacterial drugs and strengthen bacterial culture and drug resistance monitor and summarize the application characteristics of antibacterial drugs in different stages to provide a reference for the rational and standardized application of antibacterial drugs.
