反复种植失败合并慢性子宫内膜炎患者不同治疗方案的疗效观察

摘　　要：目的探讨反复种植失败合并慢性子宫内膜炎患者不同治疗方案的疗效及治疗后对妊娠结局的影响。方法回顾性选择2016年1月~2018年12月在淮安市妇幼保健院生殖中心因反复种植失败行宫腔镜检查后诊断合并慢性子宫内膜炎的患者,按排除标准排除后选取100例患者根据不同治疗方案分为四组,组1(n=30):未做任何治疗;组2(n=20):口服多西环素;组3(n=30):抗生素宫腔灌注;组4(n=20):口服多西环素联合抗生素宫腔灌注;比较不同治疗组治疗后月经血中促炎因子水平变化情况及妊娠结局情况。结果经不同方案治疗后,组3(60%)和组4(60%)的临床妊娠率明显高于组1(26.67%)和组2(30%),差异有统计学意义(P <0.05)。结论抗生素宫腔灌注治疗慢性子宫内膜炎可改善宫腔环境,提高临床妊娠率。Objective To explore the efficacy of different treatment options for patients with repeated implantation failure and chronic endometritis and the effect on pregnancy outcome after treatment.Methods Retrospectively select patients diagnosed with chronic endometritis after hysteroscopy due to repeated implantation failure in the Reproductive Center of Huai'an Maternal and Child Health Hospital from January 2016 to December 2018,and 100 patients were selected according to the exclusion criteria.According to different treatment plan,those patients were divided into 4 groups,group 1(n=30):no treatment;group 2(n=20):oral doxycycline;group 3(n=30):antibiotic intrauterine infusion;group 4(n=20):oral doxycycline combined with antibiotic intrauterine infusion;The pro-inflammatory factor levels in menstrual blood and pregnancy outcomes in different treatment groups after treatment were compared.Results After different treatments,the clinical pregnancy rate of group 3(60%)and group 4(60%)was significantly higher than that of group 1(26.67%)and group 2(30%),the difference was statistically significant(P<0.05).Conclusion Antibiotic intrauterine infusion treatment of chronic endometritis can improve the uterine cavity environment and increase the clinical pregnancy rate.
